摘 要:目的评价二巯丙磺钠(DMPS)用量与肝豆状核变性(Wilson′s disease,WD)患者血常规异常的相关性。方法采用回顾性分析的方法,查阅整理2014~2018年间某院主要诊断为"肝豆状核变性"患者的病历资料,并用Naranjo不良反应计分法和Pearson相关性分析法,评价了DMPS与血常规异常相关程度,血常规异常与基线值、用药量的相关性。结果 56例患者纳入研究,使用DMPS后,白细胞计数(WBC)、中性粒细胞计数(N)、血小板计数(PLT)较治疗前均有显著降低(P<0.05),DMPS导致的血常规异常率为83.93%。年龄、性别、WBC及N的基线值对血常规异常的影响无统计学意义(P>0.05)。血常规指标PLT基线值对血常规异常的影响有统计学意义(P<0.05)。DMPS用药量与WBC、N、PLT变化差值呈弱相关(0.2<|r|<0.4)。结论 DMPS用量与患者血常规异常之间存在一定相关性,且用药量(日剂量)越高,血常规异常越明显。年龄和性别因素并不影响血常规。因此,WD患者在临床应用DMPS的过程中需严密监测血常规。OBJECTIVE To evaluate the correlation between sodium dimercaptosulphonate(DMPS) and abnormal blood routine in patients with Wilson′s disease(WD). METHODS A retrospective analysis method was used. The records of patients with WD in a Hospital from 2014 to 2018 was collected and analyzed. Naranjo ADR probability scale and Pearson correlation analysis were used to evaluate the correlation between DMPS and the abnormal routine blood indexes. The two methods also were used to evaluate the correlation between the abnormal routine blood indexes and the baseline value and the dosage. RESULTS 56 patients were included into this study. After using DMPS,the abnormal incidence of white blood cell count(WBC),neutrophil count(N),platelet count(PLT) were significant different compared with before medication. The abnormal rate of blood routine caused by DMPS was 83.93%. There was no significant difference between age,sex,the baseline value of WBC or N and the routine blood indexes(P>0.05). There was significant difference between the baseline value of PLT and the routine blood routine(P>0.05). The dosage of DMPS was weakly correlated with the change of WBC,N and PLT(0.2<|r|≤0.4). CONCLUSION There is a certain correlation between the dosage of DMPS and abnormal blood routine,and the higher the dosage(daily dosage),the more obvious the abnormal blood routine. Age and sex do not impact the blood routine. So we should pay attention to monitor blood routine in clinical application of DMPS.
