泼尼松联合吗替麦考酚酯治疗血管炎型IgA肾病的临床疗效

摘　　要：目的探讨泼尼松联合吗替麦考酚酯治疗血管炎型免疫球蛋白A(IgA)肾病的临床疗效。方法 60例血管炎型IgA肾病患者,随机分为研究组和对照组,每组30例。研究组采用吗替麦考酚酯联合泼尼松治疗,对照组采用泼尼松联合注射用环磷酰胺治疗。观察比较两组患者治疗前后的24 h尿量、24 h尿蛋白定量、总蛋白、白蛋白、血肌酐水平以及临床疗效。结果治疗后,两组24 h尿量、24 h尿蛋白定量、总蛋白、白蛋白、血肌酐水平均优于治疗前,且研究组的24 h尿量(1963.5±189.6)ml、总蛋白(69.3±3.0)g/L、白蛋白(31.1±2.5)g/L均高于对照组的(1510.5±129.2)ml、(59.2±3.6)g/L、(22.0±3.1)g/L,24 h尿蛋白定量(1137.9±212.3)mg、血肌酐(109.8±33.1)μmol/L均低于对照组的(2926.7±286.4)mg、(261.2±45.1)μmol/L,差异均有统计学意义(P<0.05)。研究组总有效率100.0%显著高于对照组的76.7%,差异有统计学意义(P<0.05)。结论泼尼松联合吗替麦考酚酯治疗血管炎型IgA肾病具有较好的临床疗效,并能有效改善24 h尿量、24 h尿蛋白定量、白蛋白、总蛋白、血肌酐等相关指标。Objective To discuss the clinical efficacy of prednisone combined with mycophenolate mofetil in the treatment of vasculitis type immunoglobulin A(IgA) nephropathy. Methods A total of 60 patients with vasculitis type IgA nephropathy were randomly divided into research group and control group, with 30 cases in each group. The research group was treated by prednisone combined with mycophenolate mofetil, and the control group was treated by prednisone combined with cyclophosphamide for injection. The 24 urine volume, 24 h urinary protein quantification, total protein, albumin, serum creatinine level before and after treatment, and clinical effect was observed and compared between the two groups. Results After treatment, the 24 urine volume, 24 h urinary protein quantification, total protein, albumin, serum creatinine level in the two groups was better than those before treatment. The 24 urine volume(1963.5±189.6) ml, total protein(69.3±3.0) g/L, albumin(31.1±2.5) in the research group was higher than(1510.5±129.2) ml,(59.2±3.6) g/L and(22.0±3.1) g/L in the control group, and 24 h urinary protein(1137.9±212.3) mg and serum creatinine level(109.8±33.1) μmol/L was lower than(2926.7±286.4) mg and(261.2±45.1) μmol/L in the control group. Their difference was statistically significant(P<0.05). The total effective rate 100.0% in the research group was significantly higher than 76.7% in the control group, and the difference was statistically significant(P<0.05). Conclusion Prednisone combined with mycophenolate mofetil shows good clinical efficacy in the treatment of vasculitis type IgA nephropathy, and it can effectively improve related indicators of 24 urine volume, 24 h urinary protein quantification, total protein, albumin, serum creatinine.
