摘　　要：目的了解宜昌市城区一般人群带状疱疹(herpes zoster,HZ)发病率,为HZ疾病负担及疫苗研究提供基础。方法依托宜昌市健康大数据平台,对2016年宜昌市城区大数据服务系统覆盖人口开展HZ病例调査。利用城区及湖北省常住人口数计算发病率并标化。组间差异比较采用χ^2检验。结果本研究覆盖2016年城区人口783555人,报告HZ病例4600例。HZ粗发病率为5.871‱(95%C7:5.868‱~5.873‱),标化发病率为5.80‱(95%CI:5.80‱~5.81‱)。女性发病率明显高于男性,差异有统计学意义(χ^2=17.92,P<0.05)<,HZ的发病率在50岁以后明显增加(χ^2=16.82,P<0.05),50~岁组为8.40‱(959/114190),60~岁组为10.03‱(999/99636),70岁以上最高,为16.09‱(943/58614),50岁以上病例占总病例数的63.07%(2901/4600)。50岁以下患者并发疱疹后神经痛的发病率(16.30%)明显高于50岁以上患者的发病率(10.30%)(χ^2=184.68,P<0.05)。结论HZ的发病主要集中在50岁以上人群。Objective To study the community-based herpes zoster(HZ)incidence in Yichang urban area.Methods Based on the Health Big-data System in Yichang city urban area,the HZ cases enrolled in this study were clinically diagnosed cases,incidence and standardized incidence were calculated.Chi-square test was used to compare the difference of age and gender groups.Results The Health Big-data System covered 783555 population and 4600 HZ cases during the year of 2016 in this study.The crude HZ incidence was 5.87χ^(95%CI:5.868‱~5.873‱),and the standardized incidence was 5.80‱(95%CI:5.80‱~5.81‱).The incidence of females was higher than that of males(χ^2=17.92,P<0.05).Most cases happened in elder age groups.The incidences of HZ in 50-,60-,and over 70 years age group are 8.40‱,10.03‱ and 16.09‱,respectively;63.07% cases were those elder than 50 years.The postherpetic neuralgia(PHN)complication was common in HZ cases.The incidence of PHN in HZ cases of younger ages was 16.30%,which is much higher than 10.30% of elder than 50 years(χ^2=184.68,P<C0.05).Conclusions Most of HZ happened in the population over 50 years old.
