摘 要:目的:探讨小儿腹股沟疝修补术围术期预防用抗菌药物与手术切口的感染情况。方法:收集我院2011-2015年行腹股沟疝修补术的3851例患儿病历资料,在对2011年腹股沟疝修补术围术期预防用抗菌药物情况进行回顾性分析的基础上,制定"清洁手术围术期不预防使用抗菌药物方案",普外科临床医师严格执行方案的相关规定,同时监测术后手术切口感染情况,评估小儿腹股沟疝修补术围术期预防用抗菌药物对手术切口感染的影响。结果:2011-2015年各年份小儿腹股沟疝修补术围术期抗菌药物预防使用率分别为100.00%、50.06%、0.00%、0.00%、0.00%,其手术切口感染率分别为0.26%、0.26%、0.13%、0.13%、0.26%,2012-2015年手术切口感染率与2011年比较差异无统计学意义(P>0.05),符合《医院感染管理规范(试行)》中清洁手术切口感染率<0.5%的规定。结论:小儿腹股沟疝修补术围术期不预防用抗菌药物,不会导致手术切口感染率增加,可不用抗菌药物预防感染。Objective:To probe into the perioperative prophylaxis and surgical incision infection in children undergoing inguinal hernia repair.Methods:Medical records of 3,851 children undergoing inguinal hernia repair in our hospital from 2011 to 2015 were collected.Based on the retrospective analysis of the situation of prophylactic use of antibiotics in inguinal hernia repair during perioperative period in 2011,the program of No Prophylactic Use of Antibiotics during Perioperative Period of Cleaning Surgery was formulated.The clinicians of general surgery strictly implemented the relevant provisions of the program,monitored the postoperative incision infection,and evaluated the effects of antibiotics on the incision infection of inguinal hernia repair in children during perioperative period.Results:From 2011 to 2015,the prophylactic use rate of antibiotics in inguinal hernia repair during perioperative period were respectively 100.00%,50.06%,0.00%,0.00% and 0.00%.The infection rate of surgical incision were respectively 0.26%,0.26%,0.13%,0.13% and 0.26%,there was no significant difference in the infection rate of surgical incision between 2012 to 2015 and 2011(P>0.05),live up to the regulations of Nosocomial Infection Management Standard(Trial)with the infection rate of clean surgical incision<0.5%.Conclusion:Pediatric inguinal hernia repair without prophylactic use of antibiotics will not lead to the increased infections in surgical incision.
