温州市2016—2017年法定传染病报告质量分析

摘　　要：目的分析温州市法定传染病报告质量和管理现状,为进一步提高传染病报告水平提供依据。方法采用分层随机抽样方法在温州市辖区内42家机构开展现场调查,用率、构成比等指标对样本特征进行描述,率的比较采用χ2检验。结果共抽查温州市12个区县42家机构法定传染病1 559例,报告率为99.36%,及时报告率为99.97%,完整率为90.56%,准确率为99.39%,卡片录入一致率为93.33%,身份证号完整率为96.11%。不同级别医疗机构的报告率和不同类别病种的报告率差异均有统计学意义(P<0.05),乙类报告率高于丙类。不同级别医疗机构的完整率、一致率和身份证号完整率差异均有统计学意义(P<0.05)。报告质量综合指数由高到低依次为市级、省级、乡镇级和县区级。疾控机构和医疗机构的传染病报告管理现状尚不完善。结论温州市2016-2017年传染病报告质量总体情况较好,但仍存在漏报和报告不及时等问题,应继续加强传染病报告管理工作,完善医疗机构HIS系统,加强培训和监管力度,进一步提高传染病报告质量。Objective To analyze the quality and management of notifiable infectious disease in Wenzhou and provide the evidence for improving the infectious disease reports quality. Methods A total of 42 institutions were sampled through multistage stratified sampling.Sample characteristics were described by rate,composition ratio and other indicators and χ2 test was used to compare the rates.Results A total of 1 559 cases of notifiable infectious diseases from the 42 institutions in 12 districts and counties were investigated,with the overall reporting rate,timeliness rate,completeness rate,accuracy rate,concordance rate and the ID card number completeness rate of 99.36%,99.97%,90.56%,99.39%,93.33% and 96.11% respectively.There were significant differences in the reporting rate of different levels of medical institutions and different types of diseases(P<0.05) and the reporting rate of class B was higher than that of class C.There were significant differences in the completeness rate,consistency rate and identity card number completeness rate among different levels of medical institutions(P<0.05).The comprehensive index of reporting quality from high to low ranked by city level,provincial level,township level and county level.The present situation of infectious disease reporting management in CDCs and medical institutions was not perfect.Conclusion The overall situation of reporting quality of infectious diseases in Wenzhou from 2016 to 2017 is good,but there are still some problems such as missing reports and untimely reports.It should continue to strengthen the management of reporting infectious diseases,improve the HIS system of medical institutions,strengthen training and supervision,and further improve the quality of reporting infectious diseases.
