CDV联合CiE方案对晚期神经母细胞瘤患儿的疗效

摘　　要：目的探讨环磷酰胺+柔红霉素+长春新碱(CDV方案)与顺铂+足叶乙苷(CiE方案)联合治疗晚期神经母细胞瘤患儿的疗效。方法回顾性分析2011年9月至2014年8月开封市儿童医院收治的64例神经母细胞瘤晚期患儿临床资料。所有患儿均经CDV、CiE联合治疗,共6个疗程。比较患儿化疗前后血清神经元特异性烯醇化酶(NSE)、血清乳酸脱氢酶(LDH)、血清铁蛋白(SF)、24 h尿香草扁桃酸(VMA)水平,统计近期疗效及远期生存情况。结果化疗后患儿NSE、LDH、SF、24 h VMA水平均较前降低,差异有统计学意义(均P<0.05)。化疗后21例患儿(32.81%)完全缓解,34例(53.13%)部分缓解,6例(9.38%)疾病稳定,3例(4.69%)疾病进展,治疗总有效率为85.94%(55/64)。患儿中位生存时间为25.10个月,1、2、3、4年生存率分别为75.00%、53.13%、32.81%、28.13%。结论CDV联合CiE方案可有效改善晚期神经母细胞瘤患儿血清NSE、LDH、SF及24 h VMA水平,疗效较好,患儿生存率高。Objective To explore effects of cyclophosphamide+daunorubicin+vincristine(CDV)combined with cisplatin+etoposide(CiE)on advanced neuroblastoma in children.Methods The clinical data of 64 children with advanced neuroblastoma in Kaifeng Children’s Hospital from September 2011 to August 2014 were retrospectively analyzed.All children were treated with CDV and CiE for a total of 6 courses.The levels of serum neuron-specific enolase(NSE),serum lactate dehydrogenase(LDH),serum ferritin(SF),and vanillyl mandelic acid(VMA)in children were compared before and after chemotherapy.Short-term efficacy and long-term survival were observed.Results After chemotherapy,the levels of NSE,LDH,SF,and 24 h VMA in children were lower than before,and the differences were statistically significant(all P<0.05).After chemotherapy,there were 21 cases(32.81%)with complete remission,34 cases(53.13%)with partial remission,6 cases(9.38%)with stable disease,and 3 cases(4.69%)with disease progression.The total effective rate was 85.94%(55/64).The median survival time was 25.10 months.The 1-year,2-year,3-year and 4-year survival rates were 75.00%,53.13%,32.81%,and 28.13%,respectively.Conclusion The combination of CDV and CiE in treatment of advanced neuroblastoma could improve NSE,LDH,SF and 24 h VMA levels in children with advanced neuroblastoma,with good curative effect and high survival rate.
