醒脑开窍针刺联合五子衍宗汤治疗脑梗死后血管性痴呆临床研究

摘　　要：目的研究醒脑开窍针刺联合五子衍宗汤治疗脑梗死后血管性痴呆的临床疗效。方法选取2018年3月-2019年3月我院脑梗死后血管性痴呆患者86例,按随机数字表法分试验组(43例)、参照组(43例)。参照组采用常规西药(丁苯酞软胶囊)治疗,试验组在参照组基础上采用醒脑开窍针刺联合五子衍宗汤治疗。比较2组总有效率、治疗前后血清金属基质蛋白酶-9(MMP-9)、基质金属蛋白酶组织抑制因子-1(TIMP-1)水平、认知功能、日常生活能力。结果试验组总有效率为95.35%(41/43),高于参照组76.74%(33/43)(P<0.05);治疗后试验组血清MMP-9水平低于参照组,TIMP-1水平高于参照组(P<0.05);治疗后试验组MMSE、ADL评分均高于参照组(P<0.05)。结论醒脑开窍针刺联合五子衍宗汤治疗脑梗死后血管性痴呆的临床疗效显著,可有效抑制患者脑神经损伤,改善其认知功能及日常生活能力。Objective To study the clinical effect of Xingnao Kaiqiao acupuncture combined with Wuzi Yanzong decoction in the treatment of vascular dementia after cerebral infarction. Methods 86 patients with vascular dementia after cerebral infarction from March 2018 to March 2019 were enrolled. The patients were divided into the experimental group(n=43) and the control group(n=43) according to the random number table method. The control group was treated with conventional western medicine(butylphthalide soft capsule), and the experimental group was treated with Xingnao Kaiqiao acupuncture combined with Wuzi Yanzong decoction on the basis of the control group. The total effective rate, serum metalloproteinase-9(MMP-9), tissue inhibitor of metalloproteinase-1(TIMP-1) levels, cognitive function, and daily living ability were compared before and after treatment. Results The total effective rate of the experimental group was 95.35%(41/43), which was higher than that of the control group(76.74%(33/43))(P<0.05). After treatment, the serum MMP-9 level of the experimental group was lower than that of the control group, and TIMP-1 level of the experimental group were higher than those of the control group(P<0.05). The MMSE and ADL scores of the experimental group were higher than the control group after treatment(P<0.05). Conclusion The clinical effect of Xingnao Kaiqiao acupuncture combined with Wuzi Yanzong decoction in the treatment of vascular dementia after cerebral infarction is significant, which can effectively inhibit brain nerve damage and improve its cognition function and daily living ability.
