阴式子宫疤痕妊娠病灶清除术治疗Ⅰ型剖宫产术后疤痕妊娠的效果观察

摘　　要：目的探讨阴式子宫疤痕妊娠病灶清除术治疗Ⅰ型剖宫产术后疤痕妊娠(CSP)的临床疗效。方法选取我院收治的70例Ⅰ型CSP患者,随机分为观察组和对照组各35例。对照组采用子宫动脉栓塞甲氨喋呤(MTX)联合清宫术治疗,观察组采用阴式子宫疤痕妊娠病灶清除术治疗。比较两组的手术时间、术中出血量、住院时间、治愈率、超声疤痕处包块消失时间、月经恢复时间及β-HCG转阴时间。结果与对照组比较,观察组的手术时间较长,术中出血量较少,住院时间、超声疤痕处包块消失时间和月经恢复时间较短,治愈率较高(P均<0.05)。两组患者的β-HCG转阴时间比较无统计学差异(P>0.05)。结论阴式子宫切口疤痕妊娠病灶清除术治疗Ⅰ型CSP具有操作简单、疗效显著、创伤性小等优点,值得临床推广。Objective To explore the clinical efficacy of vaginal uterine scar pregnancy focus removal in the treatment of type I cesarean scar pregnancy(CSP).Methods 70 cases of patients with type I CSP admitted to our hospital were selected and randomly divided into observation group(n=35)and control group(n=35).The control group was treated with uterine artery embolization methotrexate(MTX)combined with uterine curettage,and the observation group was treated with vaginal uterine scar pregnancy focus removal.The operation time,intraoperative blood loss,hospitalization time,cure rate,disappearance time of ultrasound scar mass,menstruation recovery time and 0-HCG conversion time were compared between the two groups.Results Compared with the control group,the operation time of the observation group was longer,the intraoperative blood loss was less,the hospitalization time,disappearance time of ultrasound scar mass and recovery time of menstruation were shorter,and the cure rate was higher(all P<0.05).No statistical difference was found in the 0-HCG conversion time bwtween the two groups(P>0.05).Conclusions Vaginal uterine scar pregnancy focus removal in the treatment of type I CSP has the advantages of simple operation,significant curative effect and less trauma,which is worthy of clinical promotion.
