补肾平肝方治疗阴虚阳亢型原发性高血压的前瞻性队列研究

摘　　要：目的观察补肾平肝方联合马来酸左旋氨氯地平片治疗阴虚阳亢型原发性高血压的临床疗效及对晨起血压和肝肾功能的影响。方法采用前瞻性队列研究方法,选取2017年9月—2018年9月于河北省中医院心血管科收住院治疗且符合入组标准的151例阴虚阳亢型原发性高血压患者作为观察对象,按自愿原则分为2组,对照组76例患者给予马来酸左旋氨氯地平片治疗,治疗组75例患者给予补肾平肝方联合马来酸左旋氨氯地平片治疗。观察2组患者治疗前及治疗14 d后中医症状积分、晨起血压及肝肾功能指标的变化,统计2组患者治疗14 d后中医证候疗效。结果治疗14 d后,2组患者的中医症状积分、晨起血压均较治疗前明显降低(P均<0.05),且治疗组均明显低于对照组(P均<0.05)。治疗前2组患者肝肾功能指标均在正常范围,治疗14 d后各指标均未见明显变化。治疗14 d后,治疗组和对照组中医证候总有效率分别为90.7%(68/75)和75.0%(57/76),治疗组明显高于对照组(P<0.05)。结论补肾平肝方联合马来酸左旋氨氯地平片治疗可显著改善阴虚阳亢型原发性高血压患者的阴虚阳亢症状,降低晨起血压,且安全。Objective It is to observe the clinical efficacy of Bushen Pinggan decoction combined with L-Amlodipine maleate in the treatment of primary hypertension of type of Yin deficiency and Yang hyperactivity and its effect on morning blood pressure and liver-kidney function.Methods A prospective cohort study was used to select 151 patients with primary hypertension of type of Yin deficiency and Yang hyperactivity who were admitted to the cardiovascular department of Hebei Provincial Hospital of Traditional Chinese Medicine from September 2017 to September 2018 and met the inclusion criteria as the subjects,and were divided into two groups on a voluntary basis.76 patients in the control group were treated with L-Amlodipine maleate,and 75 patients in the treatment group were treated with Bushen Pinggan decoction combined with L-Amlodipine maleate.The changes of TCM symptom scores,morning blood pressure,and indexes of liver-kidney function in the two groups were observed before and after The curative effects of TCM syndrome after 14 days of treatment were evaluated in the two groups.Results After 14 days of treatment,the TCM symptom scores and morning blood pressure of the two groups were significantly lower than those before treatment(P<0.05),and the scores and blood pressure in the treatment group were significantly lower than those in the control group(P<0.05).The liver and kidney function indexes of the patients in the two groups were in the normal range before treatment,and no significant change was found in all indexes after 14 days of treatment.After 14 days of treatment,the total effective rates of TCM syndromes in the treatment group and control group were 90.7%(68/75)and 75.0%(57/76),respectively,and the rates in the treatment group were significantly higher than those in the control group(P<0.05).Conclusion Bushen Pinggan decoction combined with L-Amlodipine maleate can significantly improve the symptoms of Yin deficiency and Yang hyperactivity,reduce morning blood pressure,and is safe in patients with primary
