冠心病患者血清Cys-C、ICTP、MMP2、sCD40L水平变化与冠脉病变程度的关系

摘　　要：目的探讨冠心病(CHD)患者血清胱抑素C(Cys-C)、I型胶原羧基末端肽(ICTP)、基质金属蛋白酶2(MMP2)、可溶性CD40L(sCD40L)的变化与冠脉病变程度的关系。方法选取我院确诊的148例CHD患者作为CHD组,70例健康者作为对照组,检测两组血清Cys-C、ICTP、MMP2、sCD40L水平,并将CHD组分为急性心肌梗死组(AMI组)、稳定型心绞痛组(SAP组)和不稳定型心绞痛组(UAP组)及不同冠脉病变支数进行组间比较。结果CHD组患者血清Cys-C、ICTP、MMP2、sCD40L水平高于对照组(P<0.05);SAP组、UAP组及AMI组患者血清Cys-C、ICTP、MMP2、sCD40L水平组间呈逐渐升高的趋势,组间两两比较差异均具有统计学意义(P<0.05);单支、双支及三支冠脉病变的CHD患者血清Cys-C、ICTP、MMP2、sCD40L水平及Gensini评分组间呈逐渐升高的趋势,组间两两比较差异均具有统计学意义(P<0.05);CHD患者血清Cys-C、ICTP、MMP2、sCD40L水平与Gensini评分均呈显著正相关(P<0.05)。结论CHD患者血清Cys-C、ICTP、MMP2、sCD40L水平较健康人群显著升高,可能与冠脉狭窄程度及CHD患者病情加重有关。Objective To investigate the relationship between the changes of serum cystatin C(Cys-C),type I collagen carboxy terminal peptide(ICTP),matrix metalloproteinase 2(MMP2),soluble CD40L(sCD40L)and the severity of coronary artery lesion in patients with coronary heart disease(CHD).Methods 148 patients with CHD(CHD group)diagnosed in our hospital and 70 healthy subjects(control group)were selected.The serum levels of Cys-C,ICTP,MMP2 and sCD40L were detected in two groups,and CHD patients were divided into acute myocardial infarction group(AMI group),stable angina group(SAP group)and unstable angina group(UAP group),and different coronary lesion counts.Compared the level of serum Cys-C,ICTP,MMP2 and sCD40L among groups.Results The levels of serum Cys-C,ICTP,MMP2 and sCD40L in CHD group were higher than those in the control group(P<0.05).The serum Cys-C,ICTP,MMP2 and sCD40L levels in SAP group,UAP group and AMI group gradually increased.The trend of elevation was statistically significant(P<0.05).Serum Cys-C,ICTP,MMP2,sCD40L levels and Gensini scores in CHD patients with single,double and triple coronary lesions were gradually increased.There was statistically significant difference between each two groups(P<0.05).There was significantly positive correlation between serum Cys-C,ICTP,MMP2,sCD40L levels and Gensini scores in CHD patients(P<0.05).Conclusion The levels of serum Cys-C,ICTP,MMP2 and sCD40L in patients with CHD are significantly higher than those in healthy people,which may be related to the degree of coronary stenosis and the severity of CHD patients.
