摘　　要：目的:分析不孕症患者血清游离脂肪酸(FFA)、抗穆勒氏管(AMH)等指标的变化及关联性,探讨FFA对卵巢功能的影响。方法:选择2017年11月~2019年5月在武汉大学中南医院生殖医学中心就诊的且确诊为不孕症患者56例,以35岁为节点,分为适龄不孕组(30例)和高龄不孕组(26例)。筛选同期来妇产科就医孕前检查为正常、后确认受孕成功者45例作为对照组,测定并比较各组受检者的血清AMH水平,以及血清FFA、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)血脂指标水平。结果:高龄不孕组、适龄不孕组血清FFA水平(762.8±254.3μmol/L;894.9±491.2μmol/L)均高于对照组(591.9±163.2μmol/L),差异有统计学意义(P<0.05);高龄不孕组、适龄不孕组血清AMH水平(3.73±2.09ng/ml;2.77±1.82ng/ml)均低于对照组(4.01±1.73 ng/ml),但高龄不孕组差异有统计学意义(P<0.05),适龄不孕组差异无统计学意义(P>0.05);受检不孕症患者血清FFA水平与AMH等各检测指标不具相关性(P>0.05)。结论:血清FFA与AMH的水平变化无关联性,但血清FFA水平持续增高对不孕症的发生具有影响作用,是独立的影响因子。Objective:To analyze the levels and correlation of serum free fatty acid(FFA)and anti-mullerian hormone(AMH)in infertility patients,and to explore the effect of FFA on the ovarian function.Methods:56 infertility patients diagnosed at the reproductive medicine center in Zhongnan Hospital of Wuhan University from November in 20167 to May in 2019 were enrolled in this study and divided into the young group(n=30)and the elderly group(n=26)based on the age of 35-year-old.45 cases of pregnant women with normal pre-pregnancy physical examination and confirmation of pregnancy in the department of obstetrics and gynecology of our hospital during the same period were selected as the control group.Serum AMH levels,serum FFA,triglyceride(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C)and low density lipoprotein cholesterol(LDL-C)were measured and compared in each group.Results:Compared to the control group,the levels of serum FFA in elderly group(762.8±254.3μmol/L)and young group(894.9±491.2μmol/L)were significantly higher(P<0.05).Conversely,the serum AMH level in elderly group and young group(3.73±2.09ng/ml;2.77±1.82ng/ml)were lower than those in control group(4.01±1.73 ng/ml).However,there was a statistically significant difference in the elderly group(P<0.05),while there was no statistically significant difference in the young group(P>0.05).Moreover,serum FFA level in infertility patients was not correlated with AMH and other indicators(P>0.05).Conclusion:There was no correlation between serum FFA and AMH,but the continuous increase of serum FFA had an effect on the occurrence of infertility and was an independent influencing factor.
