摘 要:目的探讨海洛因成瘾者(HA)体素水平脑网络中心度(DC)的变化特点,并分析其与海洛因使用总量的相关关系.方法纳入37例HA为病例组和33例健康被试(HC)为对照组,采用3.0T MR对被试进行大脑rs-fMRI扫描,数据经后处理软件(dpabi)校正分析,获得HA组和HC组大脑DC差异图,并对差异DC脑区信号强度与患者使用海洛因总量进行偏相关性分析.结果与HC组相比,HA组双侧丘脑,右侧尾状核、右侧壳核,左侧脑岛、左侧背外侧前额叶、左侧中央前回DC值明显降低;双侧楔前叶、右侧顶上回以及左侧小脑第八区显著增高(P<0.001,最小体素>20,Alphasim校正).其中,右尾状核DC值与患者海洛因使用总量负相关(r=-0.391,P=0.017).结论HA大脑网络中背侧纹状体、抑制性控制环路、内感受中心脑区的重要节点脑区活动性下降;成瘾习惯行为的形成与吸毒总量呈正性相关.Objective To explore the characteristics of degree centrality(DC)in heroin addicts(HA)brain network in the voxel level and to analyze the correlation between them and the total amount of heroin consumption.Methods 37 HA were enrolled into the case group and 33 healthy control(HC)were recruited into the control group.rs-fMRI data was collected using a 3.0T MR scanner.The data was corrected by post processing software(dpabi)to obtain brain DC difference in HA and HC groups.The partial correlation was performed between the signal intensity within different DC brain areas and the total amount of heroin consumption.Results Compared with the HC group,HA had a significantly decreased DC in the bilateral thalamus,right caudate,right putamen,left insula,left dorsal lateral prefrontal cortex,left precentral gyrus and increased DC in the bilateral precuneus,right parietal gyrus and cerebellum-8 area(P<0.001,voxel size>20,Alphasim correction).Meanwhile,there was a negative correction between the DC value of the right caudate and the accumulated heroin consumption(r=-0.391,P=0.017).Conclusion For HA,activity of important nodes in the dorsal striatum circuitry,the inhibitory control circuitry as well as interoceptive system showed decline;The formation of addiction habits is positively related to the total amount of heroin consumption.
