微波消融联合经皮椎体成形术治疗椎体转移性肿瘤的短期疗效

摘　　要：目的探讨微波消融(MWA)联合经皮椎体成形术(PVP)治疗椎体转移性肿瘤的短期疗效,评估其安全性.方法对35例椎体转移性肿瘤的患者进行MWA联合PVP治疗,并对所有患者术前、术后第3天、术后3个月和术后6个月的疼痛及镇痛药物使用情况进行评分和比较,记录术中、术后出现的不良反应.结果35例患者[平均年龄(63.5±7.4)岁]、共42个椎体均顺利完成了MWA联合PVP.与术前相比,术后第3天视觉模拟量表(VAS)评分从(7.91±1.20)分降至(2.91±0.89)分,镇痛药使用评分从(3.20±0.87)分降至(0.89±0.96)分,且术后6个月内的VAS及镇痛药使用评分保持在较低水平,差异均有统计学意义(P<0.01).有8例患者出现骨水泥渗漏现象(22.86%),但均无相关临床症状.结论MWA联合PVP治疗胸腰椎转移瘤安全、有效,可缓解疼痛症状.Objective To investigate the short term efficacy of microwave ablation(MWA)combined with percutaneous vertebroplasty(PVP)in the treatment of vertebral metastatic tumors,and to evaluate its safety.Methods 35 patients with vertebral metastatic tumors were treated with MWA combined with PVP.The pain and the use of analgesics before,3 days after,3 months after and 6 months after operation were scored and compared,and the adverse reactions during and after operation were recorded.Results 35 patients[mean age(63.5±7.4)years]with 42 vertebral bodies scessfully completed MWA combined with PVP.C ompared with pre operation,the three day visual analog scale(VAS)score decreased from(7.91±1.20)to(2.91±.89)and the analgesic use score decreased from(3.20±0.87)to(0.89±0.96),and the VAS and analgesic use score remained at a low level within 6 months after operation,with statistically significant differences(P<0.01).8 patients had bone cement leakage(22.86%),but no related clinical symptoms.Conclusion MWA combined with PVP for the treatment of thoracolumbar metastases is safe and effective,which can relieve pain symptoms.
