摘 要:目的探讨瑞芬太尼预处理对老年大鼠脑缺血再灌注(I/R)损伤血小板衍生因子(PDGF)、髓鞘碱性蛋白(MBP)表达的影响,探讨其脑保护作用机制。方法将大鼠随机分为空白组,假手术组,I/R组,瑞芬太尼预处理组4组。除空白组外,各组均按大鼠脑缺血2 h后,再灌注时间6、12、24 h分为3个亚组,每组5只。造模成功后取大鼠缺血侧脑组织,采取免疫组化法检测PDGF、MBP表达,并行苏木素-伊红(HE)染色镜下观察脑组织病理形态。结果HE染色:空白组、假手术组脑组织病理形态无明显改变;I/R组神经元结构明显受损,瑞芬太尼预处理组较I/R组在各时间点神经元受损减轻。免疫组化:与空白组、假手术组、I/R组比较,瑞芬太尼预处理组PDGF阳性细胞表达在各时间点均明显升高(P<0.01);I/R组MBP阳性细胞表达数与其他各组比较明显升高(P<0.01)。结论瑞芬太尼预处理可以减轻大鼠脑I/R损伤程度,抑制神经元坏死和凋亡,发挥脑保护作用。Objective To investigate the effect of remifentanil pretreatment on the expression of PDGF and MBP in aged rats by focal cerebral ischemia reperfusion(I/R)so as to investigate cerebral protection mechanism of remifentanil.Methods All SD rats were randomly divided into the normal control,sham operation,I/R and remifentanil pretreatment groups.Except the normal control group,rats in other groups were induced acute cerebral ischemia for 2 h and were divided into 6 h,12 h and 24 h subgroups after reperfusion.The brain were stained by HE and observed with optical microscopes.The changes of PDGF and MBP expression on the ischemic side of hippocampus were measured by immunohistochemistry.Results HE staining:①Brain tissue of the normal control group and sham operation group had no significant change in pathological morphology.②Compared with I/R group,the remifentanil pretreatment group damage of nerve cell was reduced at all time.Immunohistochemistry:①Compard with the the normal control group,sham operation group,I/R group,the expression of PDGF increased significantly at all time(P<0.01)in the remifentanil pretreatment group.②Compared with other groups,the expression of MBP significantly increased in I/R group(P<0.01).Compared with the I/R group,the expression of MBP in the remifentanil pretreatment group remarkably decreased at 6,12,24 h.Conclusions Remifentanil pretreatment could reduce cerebral ischemia reperfusion injury and play a role of brain protection.
