二至天癸颗粒对高龄肾气阴两虚IVF-ET患者Mfn2表达的影响:随机对照研究

摘　　要：目的观察二至天癸颗粒对高龄肾气阴两虚行体外受精-胚胎移植(IVF-ET)患者卵巢颗粒细胞线粒体融合素蛋白(Mfn2)及线粒体膜电位的调控作用。方法将86例行IVF-ET治疗的高龄肾气阴两虚证患者单盲随机分为中药治疗组和安慰剂组各43例。2组均采用拮抗剂方案(GnRH-ant方案)降调节至人绒毛膜促性腺激素(hCG)注射日。中药治疗组于IVF-ET治疗前3个周期始,自月经第3天起给予二至天癸颗粒。取卵日收集卵泡液并提取颗粒细胞,分析各组患者妊娠结局,检测Mfn2 mRNA表达及蛋白的含量和线粒体膜电位(△Ψm)的变化。结果中药治疗组、安慰剂组的临床妊娠率分别为42.1.%(16/38)、26.3%(10/38),中药治疗组与安慰剂组比较差异无统计学意义(P>0.05),但有升高的趋势。与安慰剂组比较,中药治疗组Mfn2 mRNA的相对表达量升高(P<0.05,P<0.01),Mfn2蛋白表达量及△Ψm荧光强度比值升高(P<0.01)。结论二至天癸颗粒可提高高龄肾气阴两虚型IVF-ET患者卵泡液颗粒细胞中Mfn2及线粒体膜电位的表达,对高龄肾气阴两虚型IVF-ET患者临床妊娠率有升高趋势。Objective To observe the regulatory effect of Erzhi Tiangui Granule on ovarian granulosa cell Mfn2 in advanced maternal age patients undergoing in vitro fertilization-embryo transfer(IVF-ET)with deficiency of Shen qi and yin.Methods Totally 86 patients underwent IVF-ET with Shen qi and yin deficiency were randomly and equally assigned to TCM treatment group and placebo group,43 in each group.In both groups,the antagonist regimen(GnRH-ant regimen)was used until the injection date of human chorionic gonadotropin(hCG).TCM treatment group was given Erzhi Tiangui Granule from the third day of menstruation before 3 cycles of IVF-ET treatment.On the day of oocyte retrieval,follicular fluid and extracted granulosa cells were collected,then the content of mitofusin 2(Mfn2)mRNA and Mfn2 protein were analyzed,and the change of △Ψm was detected.Results The clinical pregnancy rate of the TCM treatment group was 42.1%(16/38)and the placebo group was 26.3%(10/38).There was no statistically significant difference between the TCM treatment group and the placebo group(P>0.05),but there was an increasing trend between them.Compared with the placebo group,the expression of Mfn2 mRNA was increased in the TCM treatment(P<0.05,P<0.01),the Mfn2 protein expression and△Ψm higher fluorescence intensity ratio were enhanced(P<0.01).Conclusions Erzhi Tiangui Granule may improve the expression of Mfn2 and mitochondrial membrane potential in follicular fluid granule cells of elderly IVF-ET patients with deficiency of Shen qi and yin.To some extent,it can explain the beneficial trend of increasing pregnancy rate in advanced maternal age IVF-ET patients with deficiency of Shen qi and yin.
