摘 要:目的探讨人白介素-10(hIL-10)介导核转录因子p65(NF-κB p65)减轻呼吸机相关性肺损伤(VILI)及其可能机制。方法45只SD大鼠,随机分为三组:对照组(Control组)、大潮气量机械通气组(VILI组)和大潮气量机械通气加hIL-10处理组(hIL-10组),其中Control组经口插管后维持自主呼吸4h;VILI组采用大潮气量(VT)(VT:40mL·kg-1)机械通气建立大鼠VILI模型;hIL-10组机械通气开始前30min从尾静脉注射hIL-10(剂量为8000U·kg-1)30μL,Control组、VILI组给予等量的生理盐水。各组于通气4h后,采大鼠腹主动脉血样做血气分析,测定氧合指数(PaO2/FiO2);右中叶肺组织称湿重后65℃烘烤至恒重,称干重,计算湿干重比(W/D);右下叶新鲜肺组织制备成匀浆,Western blot法检测NF-κB P65的含量。结果机械通气4h后,VILI组、hIL-10组大鼠PaO2/FiO2较Control组明显降低,W/D明显升高;与VILI组相比,hIL-10组大鼠PaO2/FiO2明显升高,W/D明显降低。与Control组相比,VILI组、hIL-10组大鼠NF-κB p65蛋白表达均明显增加;与VILI组相比,hIL-10组大鼠NF-κB p65的表达明显减少。结论hIL-10可减轻大鼠呼吸机相关性肺损伤,其可能机制与介导核转录因子P65信号通路有关。OBJECTIVE To investigate the possible mechanism of human interleukin-10(hIL-10)mediated nuclear transcription factor p65(NF-κB p65)for the reduction of ventilator-induced lung injury(VILI).METHODS 45 SD rats were randomly divided into three groups:control group(Control group),high-tidal volume mechanical ventilation group(VILI group)and high-tidal volume mechanical ventilation plus hIL-10 group(hIL-10 group).Control group maintained spontaneous breathing for four hours by orally inserted tube.In VILI group,VILI model was made by mechanical ventilation of high tidal volume(VT:40 mL·kg-1);The hIL-10 group was injected with hIL-10(dose 8000 U·kg-1)30μL through tail vein at 30 minutes before the start of mechanical ventilation Control group and VILI group were given the same amount of saline.The rats of three groups were sacrificed after ventilating for four hours,the blood samples from the abdominal aorta were taken for blood gas analysis,and determining the oxygenation index(PaO2/FiO2);The right middle lung was dried to constant weight in a oven of 65 degrees celsius.Wet-to-dry weight ratio(W/D)of the lung was figured out;The fresh lung tissue of the lower right lung was prepared into a homogenized pulp,and the content of NF-κB p65 was detected by Western blot method.RESULTS Four hours after mechanical ventilation,the PaO2/FiO2 of VILI group and hIL-10 group was significantly lower than Control group,W/D was significantly higher than Control group.Compared with VILI group,the PaO2/FiO2 of group hIL-10 increased significantly,the W/D decreased significantly.Compared with Control group,the expression of NF-κB p65 protein in VILI group and hIL-10 group was significantly increased.Compared with VILI group,the expression of NF-κB p65 in hIL-10 group was significantly reduced.CONCLUSION hIL-10 can reduce the lung injury associated with ventilator in rats,and its mechanism may be related to the signaling pathway of nuclear transcription factor p65.
