非布司他联合枸橼酸钾缓释片治疗痛风及高尿酸血症肾结石疗效

摘　　要：目的探讨非布司他联合枸橼酸钾缓释片治疗痛风及高尿酸血症肾结石临床疗效。方法选取本院收治的78例痛风及高尿酸血症肾结石患者,随机分成两组,各39例,对照组口服非布司他片,观察组在对照组基础上口服枸橼酸钾缓释片,观察两组治疗尿液和血液实验检验指标及排石情况、肝肾功能、不良反应发生情况。结果观察组治疗后尿钾、尿枸橼酸及尿pH较治疗前增高、尿钙较治疗前降低(P<0.05);两组血清尿酸水平较治疗前均降低(P<0.05),观察组治疗后各时期血清尿酸低于同期对照组,而尿酸达标率高于同期对照组(P>0.05);观察组排石总有效率为89.74%,较对照组64.10%明显增高(P<0.05);治疗后两组肝肾功能指标差异无统计学意义(P>0.05);观察组和对照组不良反应总发生率(17.75%、28.21%)差异无统计学意义(P>0.05)。结论非布司他联合枸橼酸钾缓释片是治疗痛风及高尿酸血症肾结石患者的有效方案,具有较高的安全性,值得临床推广应用。Objective To investigate the clinical efficacy of febuxostat combined with potassium citrate sustained⁃release tablets in the treatment of gout and hyperuricemia with kidney stones.Methods A total of 78 patients with gout and hyperuricemia with kidney stones were randomly divided into 2 groups.The control group(n=39)was treated with febuxostat tablets by oral administration and the observation group(n=39)was treated with febuxostat tablets plus potassium citrate sustained⁃release tablets by oral administration.Urine and blood test indicators,stone discharge,liver and kidney function,and adverse reactions in the two groups were observed.Results In the observation group,urinary potassium,urinary citrate and urine PH increased and urinary calcium decreased after treatment than before treatment(P<0.05).Serum uric acid levels in both groups were lower after treatment than before treatment(P<0.05).Serum uric acid was lower in the observation group than in the control group and the uric acid compliance rate was higher than the control group(P>0.05).The total effective rate of the stone in the observation group was 89.74%,which was significantly higher than that of the control group(64.10%)(P<0.05).There were no significant differences in liver and kidney function indexes between the two groups after treatment(P>0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(17.75%,28.21%)(P>0.05).Conclusion Febuxostat combined with potassium citrate sustained⁃release tablets is an effective treatment for patients with gout and hyperuricemia with kidney stones.It has high safety and is worthy of clinical application.
