对《外科学》第9版骨折治疗原则的商榷

摘　　要：我国本科教材《外科学》第9版对骨折治疗原则的修订不符合逻辑:一是手法复位应争取达到解剖复位,否则必须手术复位;二是删除了切开复位指征中涉及功能复位的限制条款。本文认为其完全忽视了功能复位标准的存在价值。手法复位不成功才具手术指征,达到功能复位标准往往无须手术;功能复位标准对切开复位指征具有规则性的制约作用,不能自相矛盾地删除否定,使其成为摆设。骨折的非手术治疗和手术治疗同等重要。The revise of principle of fracture treatment is illogical in the ninth edition of Chinese teaching book Surgery.Firstly,manual reduction should realize anatomical reduction,otherwise surgical reduction is needed.Secondly,the restrictive covenants of functional reduction referring to the indication of open reduction are canceled.It neglects the value of norm of functional reduction.Unsuccessful manual reduction is an indication of surgical reduction.If the manual reduction can reach the functional reduction,the surgery is not necessary.The norm of functional reduction regularly restricts the indication of open reduction.We can’t cancel or deny these norms paradoxically,otherwise they are just decoration.The surgical and nonsurgical treatment of fracture are important equally.
