摘 要:目的:研究药师参与慢病管理对糖尿病患者血糖控制水平、依从性和不良事件发生率的影响,为建立药师参与慢病管理的药学服务模式提供参考依据。方法:采用随机、对照实验,将入组患者随机分为实验组和对照组,实验组由药师参与慢病管理,为患者提供药学干预,对照组纳入普通管理,随访6个月后,对比分析两组患者在干预前后及两组患者之间血糖控制情况、用药依从性以及药品不良反应发生率等各项指标。结果:实验组患者在药师干预前后FPG、2h PG均明显降低(P<0.01),对照组患者实验前后均无显著性差异(P>0.05),两组患者的HbA1c均无显著性差异(P>0.05);组间比较FPG、2hPG、HbA1c均无统计学意义(P>0.05);干预后实验组患者用药依从性评分明显提高(P<0.05),对照组患者变化不明显(P>0.05);实验组患者低血糖反应发生率明显低于对照组。结论:药师可为患者提供安全、合理用药的保障,药师干预后可提高糖尿病患者的临床治疗效果,药师参与慢病治疗的模式值得推广。Objective:To study the influence of pharmacist intervention on blood glucose control,compliance and incidence of adverse drug reactions(ADRs)in diabetic patients,so as to provide references for establishing pharmaceutical service model for pharmacists participating in chronic disease management.Methods:Patients enrolled in this study were randomly divided into experimental group and control group.Patients in experimental group accepted pharmaceutical interventions,while patients in control group accepted general management.After 6 months of follow-up,blood glucose control,medication compliance and incidence of ADR in two groups and those indexes before and after intervention were compared.Results:The fasting plasma glucose(FPG)and 2 hours postprandial blood glucose(2 hPG)after intervention in the experimental group were significantly decrease(P<0.01),while those in control group had no significantly change(P>0.05).There were no significant differences in FPG,2 hPG,and glycated haemoglobin(Hb A1 c)between the experimental group and the control group(P>0.05).The medication compliance score after intervention in the experimental group were significantly increased(P<0.05),while that in control group had no significantly change(P>0.05).The incidence of hypoglycemia in the experimental group was significantly lower than that in the control group.Conclusion:Pharmacists could ensure the safe and reasonable drug use for patients.Pharmaceutic intervention could improve the curative effect of patients with diabetes.It was worthy to encourage pharmacists to participate into the management of chronic diseases.
