摘 要:目的探讨血钙水平与老年高血压性脑出血患者血肿体积的关系。方法回顾性分析老年高血压性脑出血患者189例,根据入院时血钙水平分为对照组(2.1~2.7 mmol/L)142例和观察组(<2.1 mmol/L)47例,收集2组临床基线资料,采用格拉斯哥昏迷评分(GCS)进行神经功能缺损评估,头颅CT检查血肿体积及位置,采用Spearman相关性分析血钙水平与血肿体积的关系。结果与对照组比较,观察组男性比例、血肿体积、脑叶出血以及血肿破入脑室发生率明显升高,GCS明显降低,差异有统计学意义[63.83%vs 50.70%,P=0.047;33.9(13.9,58.1)cm3 vs 21.7(9.7,47.9)cm^3,P=0.001;38.30%vs 31.69%,P=0.041;48.94%vs 32.39%,P=0.000;9.0(5.0,13.0)分vs 11.0(6.0,15.0)分,P=0.001]。Spearman相关性分析显示,脑出血患者入院时GCS评分与血肿体积呈正相关(r=0.421,P=0.000),血钙水平与血肿体积呈负相关(r=-0.112,P=0.001)。结论低血钙水平与高血压性脑出血血肿体积呈负相关,血钙水平可能反映老年高血压性脑出血血肿体积和病情危重程度。Objective To study the relationship between blood calcium level and hematoma volume in elderly hypertensive intracerebral hemorrhage patients.Methods One hundred and eighty-nine elderly hypertensive intracerebral hemorrhage patients were divided into control group(n=142)and observation group(n=47)according to their blood calcium level on admission.Their baseline clinical data were recorded,their neurological defect was assessed according to their GCS score,their hematoma volume was measured and their hematoma site was located by brain CTA.The relationship between blood calcium level and hematoma volume in elderly hypertensive intracerebral hemorrhage patients was analyzed by Spearman correlation analysis.Results The ratio of male patients and incidence of lobar hemorrhage and hematoma breaking into ventricles and hematoma volume were significantly higher while the GCS score was significantly lower in observation group than in control group(63.83%vs 50.70%,P=0.047;38.30%vs 31.69%,P=0.041;48.94%vs 32.39%,P=0.000;33.9 cm3 vs 21.7cm3,P=0.001;9.0 vs 11.0,P=0.001).Spearman correlation analysis showed that the GCS score was positively related with the hematoma volume while the blood calcium level was negatively related with the hematoma volume in hypertensive intracerebral hemorrhage patients on admission(r=-0.112,P=0.001).Conclusion Blood calcium level is negatively related with hematoma volume and can thus be used as a biomarker for hematoma volume in hypertensive intracerebral hemorrhage patients.
