高血压脉象客观化临床研究的系统评价

摘　　要：目的为高血压脉象客观化的临床应用及后续研究提供循证医学依据。方法检索中国知网、万方数据、维普数据、PubMed和Web of Science,查找高血压脉象客观化的临床对照试验,运用RevMan 5.3软件进行统计学分析。结果共纳入12项研究,累计高血压病患者1503例,健康人972例。系统评价结果显示,结局指标中h1[P=0.0004,SMD 0.30,95%CI(0.13,0.46)]、h3/h1[P=0.003,SMD 0.46,95%CI(0.15,0.77)]、h5[P=0.0004,SMD 0.46,95%CI(-0.71,-0.20)]、w1/t[P<0.00001,SMD 0.39,95%CI(0.29,0.49)]等方面与对照组比较差异有统计学意义。h4/h1、t1、t3、t4及t5与对照组比较差异无统计学意义。结论本研究通过荟萃分析的方法证实了高血压患者与正常人的的脉诊信息差异客观存在,h1、h3/h1、h5及w1/t等变量可能与高血压相关,利用脉诊客观化可以为中医临床辩证诊断提供辅助依据。Objective To provide evidence-based medical evidence for the clinical application of the pulse manifestation objectification of hypertension and further studies.Methods CNKI,Wanfang Data,VIP Database,PubMed and Web of Science databases were searched to collect clinical-controlled trials of objectification of the pulse manifestation of hypertension,and RevMan5.3 software was used for statistical analysis.Results A total of 12 studies involving 1503 patients with hypertension and 972 healthy people were included.The result showed that there were significant differences between the case group and the control group in the outcome measures of h1[P=0.0004,SMD 0.30,95%CI(0.13,0.46)],h3/h1[P=0.003,SMD 0.46,95%CI(0.15,0.77)],h5[P=0.0004,SMD 0.46,95%CI(-0.71,-0.20)],w1/t[P<0.00001,SMD 0.39,95%CI(0.29,0.49)].There was no significant difference in h4/h1,t1,t3,t4 and t5 compared with the control group.Conclusion This study has confirmed the difference in the pulse diagnosis information between patients with hypertension and healthy people by meta-analysis.The variables of h1,h3/h1,h5 and w1/t are possibly associated with hypertension.The objectification of pulse diagnosis can provide reference for TCM diagnosis by syndrome differentiation.
