加味泽泻汤治疗原发性高血压病痰湿壅盛型患者的疗效观察

摘　　要：目的观察分析加味泽泻汤联合西药及西药单用治疗痰湿壅盛型高血压病的疗效。方法选取江阴市第二人民医院2018年1月—2018年8月收治的200例原发性高血压病痰湿壅盛型患者,根据随机数表法分为对照组与治疗组,各100例。对照组患者单用苯磺酸氨氯地平口服治疗,治疗组在对照组基础上联合使用加味泽泻汤,以4周为1个疗程,治疗1个疗程后,检测并比较两组患者血压水平、血脂水平变化情况,记录并比较两组治疗前、治疗4周后证候积分的变化情况;同时比较两组患者治疗疗效及安全性。结果治疗后两组原发性高血压病痰湿壅盛型患者证候总积分较治疗前均降低(P<0.05),且治疗组降低幅度高于对照组,差异有统计学意义(P<0.05)。治疗后两组原发性高血压病痰湿壅盛型患者收缩压、舒张压水平均较治疗前降低(P<0.05),且治疗组降低幅度优于对照组,差异有统计学意义(P<0.05)。治疗后两组原发性高血压病痰湿壅盛型患者总胆固醇(Totle Cholestero,TC)、三酰甘油(Triglycerde,TG)、低密度脂蛋白胆固醇(Low Density Lipoprotein Cholesterol,LDL-C)水平均较治疗前降低,高密度脂蛋白胆固醇(High-density Lipoprotein Cholesterol,HDL-C)较治疗前升高(P<0.05),且治疗组治疗后变化幅度优于对照组,差异有统计学意义(P<0.05)。治疗后治疗组总有效率97.00%高于对照组73.00%,两组比较差异有统计学意义(P<0.05)。治疗后两组患者各项常规检查均未见异常,且治疗期间无明显不良反应发生。结论加味泽泻汤与西药联合治疗痰湿壅盛型高血压病疗效确切,患者各临床症状更好更快缓解,血压控制效果好,血脂水平明显改善,短期内用药未见不良反应发生,安全性较理想。Objective To evaluate the effect of Modified Zexie Decoction combined with western medicine in the treatment of hypertension differentiated by excessive phlegm-dampness syndrome.Methods A total of 200 hypertension patients with the syndrome of excessive phlegm-dampness admitted to the Second People's Hospital of Jiangyin from March,2016 to March,2018 were included and divided into control group and traditional Chinese medicine(TCM)group by using random number table,with 100 cases in each group.The control group was given oral amlodipine besylate,while the TCM group was given Modified Zexie Decoction based on the control group for 4 weeks(one treatment course).After treatment,blood pressure,blood lipid,TCM symptom score in both groups were detected and compared.The clinical effect and safety were compared.Results After treatment,the total syndrome score decreased(P<0.05),and the improvement in the TCM group was greater than in the control group,with statistical significance(P<0.05).The systolic pressure and diastolic pressure in both groups decreased than before treatment(P<0.05),and the TCM group had larger reductions than the control group,with statistical significance(P<0.05).Both groups had an increase in total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C)and a reduction in high density lipoprotein cholesterol(HDL-C)(P<0.05),and the improvement in the TCM group was greater than in the control group,with statistical significance(P<0.05).The total effective rate in the TCM group was 97.00%,which was higher than that(73.00%)in the control group,with statistical significance(P<0.05).There was no adverser events in both groups during and after treatment.Conclusion Modified Zexie Decoction combined with western medicine in treatment of hypertension with syndrome of excessive phlegm-dampness is effective,which can relieve clinical symptoms,control blood pressure,improve blood lipid level.The short-term efficacy is safe and no adverse reactions were shown.
