通心络胶囊对老年丘脑损伤后感觉障碍患者IL-6、IL-18水平的影响

摘　　要：目的观察通心络胶囊对老年丘脑损伤后感觉障碍患者白介素-6(IL-6)、白介素-18(IL-18)水平的影响。方法选取2015年4月—2018年4月收治的80例老年丘脑损伤后感觉障碍患者,按随机数表法分为治疗组与对照组,各40例。对照组采取常规内科治疗联合康复训练,共治疗4周。治疗组在对照组基础上应用通心络胶囊,连续用药4周。比较两组治疗前、治疗4周后的感觉障碍[采用感觉障碍积分评定表(FLA)与Semmes-Weinstein单丝检测法评估]、肢体肌力(采用表面肌电图评估)、日常生活能力(采用Barthel指数评估)、IL-6、IL-18水平与治疗期间不良反应。结果治疗4周后,两组老年丘脑损伤后感觉障碍患者FLA评分均较治疗前显著升高、手足部单丝值较治疗前显著降低(P<0.05);且治疗4周后治疗组的FLA评分显著高于对照组,手足部单丝值显著低于对照组,两组比较差异有统计学意义(P<0.05)。治疗4周后,两组老年丘脑损伤后感觉障碍患者表面肌电图RMS、Barthel指数均较治疗前显著升高(P<0.05);且治疗组治疗4周后的表面肌电图RMS、Barthel指数显著高于对照组(P<0.05)。治疗4周后,两组老年丘脑损伤后感觉障碍患者IL-6、IL-18水平均较治疗前显著降低(P<0.05);且治疗组治疗4周后的IL-6、IL-18水平显著低于对照组(P<0.05)。两组治疗期间老年丘脑损伤后感觉障碍患者主诉无明显不适。结论通心络胶囊用于老年丘脑损伤后感觉障碍患者有利于改善脑部血液循环病下调血清IL-6、IL-18水平,促使患者感觉障碍获得缓解、日常生活能力获得提高。Objective To observe the effect of Tongxinluo Capsules on the levels of serum interleukin-6(IL-6),interleukin-18(IL-18)in elderly patients with sensory disorder after thalamic injury.Methods A total of 80 elderly patients with sensory disorder after thalamic injury admitted to our hospital from April,2015 to April,2018 were included and randomly divided into the treatment group and the control group by random number table method,with 40 cases in each group.The control group received routine medical treatment combined with rehabilitation training for a total of 4 weeks.The treatment group was treated with Tongxinluo Capsules on the basis of the treatment of the control group for 4 weeks.Sensory disorder(assessed by FLA sensory disorder score and Semmes-Weinstein monofilament test),limb muscle strength[assessed by surface electromyography(EMG)],activities of daily living(assessed by Barthel Index),serum IL-6,IL-18 levels and adverse events in both groups were compared before and after treatment.Results After 4-week treatment,both groups had a significant increase in the FLA score and a significant reduction in the hand-foot monofilament value(P<0.05);and the treatment group had a significantly higher FLA score and significantly lower hand-foot monofilament value than the control group(P<0.05).After 4-week treatment,both groups had a significant increase in the root mean square(RMS)of surface EMG and Barthel index(P<0.05),and RMS and Barthel index in the treatment group was significantly higher than in the control group(P<0.05).After 4-week treatment,the levels of IL-6 and IL-18 decreased significantly compared with those before treatment(P<0.05);and the IL-6 and IL-18 levels in the treatment group were significantly lower than those in the control group(P<0.05).During treatment,no obvious adverse event was found in patients of either groups.Conclusion For elderly patients with sensory disorder after thalamic injury,Tongxinluo Capsules can promote cerebral blood circulation,down-regulate serum levels of IL-6 and IL-
