中西医结合保守治疗输卵管妊娠临床结局观察

摘　　要：目的探索中西医结合保守治疗输卵管妊娠的临床结局。方法将入选的111例输卵管妊娠患者随机分入对照组和治疗组,对照组给予米非司酮及甲氨蝶呤治疗,治疗组在对照组治疗的基础上加服中药治疗一周,观察两组患者临床治疗效果、输卵管通畅情况、2年后生育情况及不良反应。结果治疗后治疗组患者总有效率为92.96%,明显高于对照组的70.91%(χ^2=9.049,P=0.003);治疗后月经恢复3个月,进行输卵管造影,治疗组输卵管通畅率为82.69%,明显高于对照组的61.54%(χ^2=5.136,P=0.023);随访2年,治疗组宫内妊娠率明显高于对照组(P<0.05);两组不良反应差异无统计学意义(P>0.05)。结论中西医联合保守治疗对有生育要求的输卵管妊娠患者是较好的临床治疗方法。Objective To explore of conservative treatment of integrated traditional Chinese and western medicine on tubal pregnancy.Methods A total of 111 patients with tubal pregnancy were randomly divided into the control group and the treatment group.The control group was given mifepristone and methotrexate,while the treatment group was given traditional Chinese medicine combined with mifepristone and methotrexate for one week.The clinical effect,fallopian tube condition,fertility and adverse reactions at 2-year follow up in both groups were compared.Results The total effective rate in the observation group was 92.96%,which was significantly higher than that(70.91%)in the control group(χ^2=9.049,P=0.003).After 3 months of the recovery of menstruation,the reopen rate of the fallopian tube detected by salpingography in the treatment group was 82.69%,which was significantly higher than that(61.54%)in the control group(χ^2=5.136,P=0.023).At the 2-year follow up,the intrauterine pregnancy rate in the treatment group was significantly higher than the control group(P<0.05).There was no significant difference in the incidence of adverse events between the two groups(P>0.05).Conclusion Conservative treatment of integrated traditional Chinese and western medicine is an effective therapy for tubal pregnancy patients with pregnancy desire.
