某院600例住院患者糖皮质激素类药物临床应用专项点评分析

摘　　要：目的了解某院住院患者使用糖皮质激素类药物的现状及存在问题,并对点评结果进行分析评价,进一步提高糖皮质激素类药物用药的合理性。方法利用博繁数字化系统,随机抽取某院2018年7~12月600份使用糖皮质激素类药物的归档病例,采用回顾性分析方法,对用药品种、临床使用情况、用药合理性及用药描述等进行统计分析。结果地塞米松磷酸钠注射液为某院糖皮质激素类药物使用率最高的品种,占46.67%;儿科为临床各科室中使用糖皮质激素类药物所占构成比最高的科室,为19.83%;临床各系统疾病中使用糖皮质激素类药物所占构成比最高的为呼吸系统疾病,占35.67%;在不合理使用糖皮质激素病例中,给药途径不适宜占主导,为31.67%。结论根据600份病例的分析点评结果可初步判断:糖皮质激素类药物的临床应用基本合理,但仍存在不合理用药现象。应依据《糖皮质激素类药物临床应用指导原则》,进一步规范糖皮质激素的临床应用并加强管理,促进糖皮质激素类药物的临床合理应用,确保临床用药安全、有效。Objective To understand the current state and problems of glucocorticoids uses in inpatients,and to assess the results of the comments for improving the rationality of using glucocorticoids.Methods 600 archived cases using glucocorticoid drugs in our hospital were randomly selected in Bofan digital system from July to December 2018.Drug categories,clinical drug-usage,rationality and description of drug use were retrospectively assessed.Results Dexamethasone sodium phosphate injection was the highest utilized glucocorticoids in this hospital,accounting for 46.67%.As for the glucocorticoids uses among different clinics,pediatrics had the highest proportion,accounting for 19.83%;as high as 35.67%of the drug was used in respiratory system in clinics.31.67%of inappropriate administration was found in improper usage of glucocorticoids.Conclusion Retrospective analysis of 600 archived cases show appropriate clinical application of glucocorticoids in our hospital,yet inappropriate administration still exists.The further normalized application of glucocorticoids needs to be strengthened for the improvement of rational use,safety and effectiveness of clinical medication in accordance with the guiding principles of clinical application of glucocorticoids.
