摘　　要：目的分析静脉药物配置中心(PIVAS)用药医嘱审核结果,评价循环审方模式,探索新的用药医嘱审核模式。方法收集2016年7月至2018年12月我院PIVAS审方药师采用循环审方模式发现并登记的不合理用药医嘱,对医嘱审核结果及不合理用药原因分布进行分析。结果通过两年半的实践,随着循环审方的持续开展,我院静脉用药医嘱不合格率逐年下降。结论循环审方模式是一种较为全面、全程、有效的人工用药医嘱审核模式,在不断提高审方药师综合素质的同时,将人工循环审核与软件审核相结合是PIVAS静脉用药医嘱审核的新的探索之路。Objective To analyze the results of medication order reviewing in Pharmacy Intravenous Admixture Service(PIVAS).To evaluate the cyclical prescription-checking mode.To explore new prescription-checking mode.Methods The irrational medications that were discovered and registered by the PIVAS prescription-checking pharmacists with cyclical prescription-checking mode in our hospital from July 2016 to December 2018 were collected.The results of medication order reviewing and reasons for unreasonable prescription were analyzed.Results After two and a half years of practice,with the continuous development of cyclical prescription-checking mode,the rate of unqualified intravenous drug use in our hospital has decreased year by year.Conclusion Cyclical prescription-checking mode is a comprehensive,full-course and effective manual medication orders reviewing mode.It is a new road for exploring the medication order reviewing in PIVAS to combine manual cyclical reviewing with software reviewing while continuously improving the comprehensive quality of the prescription pharmacist.
