摘 要:目的探讨胃肠外科患者术后感染的特点及其危险因素,并提出预防对策。方法选取7293例胃肠外科患者进行回顾性调查,分析其术后感染率、感染部位及其危险因素,采用多因素Logistic回归法筛选患者术后感染的危险因素。结果7293例胃肠外科患者术后感染率为8.04%,感染部位以呼吸道感染、手术切口感染及泌尿系统感染为主;多因素分析结果显示,影响术后感染的危险因素包括年龄(OR=2.128)、合并基础疾病(OR=2.782)、切口长度(OR=3.490)、切口类型(OR=2.557)、侵入性操作(OR=4.204)、未预防使用抗生素(OR=4.860)等。结论胃肠外科患者术后感染率在各科室中处于较高水平,应结合其感染特点及其危险因素,早期采取预防措施,降低术后感染发生率。Objective To investigate the characteristics and risk factors of postoperative infection in patients with gastrointestinal surgery and to propose preventive measures.Methods A total of 7293 patients with gastrointestinal surgery were retrospectively analyzed.The postoperative infection rate,infection site and risk factors were analyzed.Multifactor logistic regression was used to screen the risk factors of postoperative infection.Results The postoperative infection rate of the 7293 patients with gastrointestinal surgery was 8.04%.The respiratory tract,surgical wound and urinary tract were the main infection sites.Multifactor analysis showed that the risk factors affecting postoperative infection included patients’age(OR=2.128),combining with underlying diseases(OR=2.782),length of incision(OR=3.490),type of incision(OR=2.557),invasive procedure(OR=4.204),antibiotic use(OR=4.860),etc.Conclusion The postoperative infection rate of patients with gastrointestinal surgery is at a high level comparing with other departments.Combined with its infection characteristics and risk factors,early preventive measures should be taken to reduce the incidence of postoperative infection.
