血清可溶性髓系细胞触发受体-1及糖基化终产物在脓毒症诊断中的临床价值

摘　　要：目的:探讨血清可溶性髓样细胞表达的触发受体-1(sTREM-1)与血清糖基化终产物(AGEs)对脓毒症患者疾病严重程度及预后的评估作用。方法:选择56例脓毒症患者和56例一般感染患者,分别于入院当天采血检测sTREM-1与AGEs的表达水平,入院后3、6 d重复检测sTREM-1与AGEs的表达水平,同时记录序贯性器官功能衰竭评分标准(SOFA评分)及患者20 d生存情况。结果:sTREM-1、AGEs在脓毒症组明显高于一般感染组,两组比较差异有统计学意义(P<0.05);在脓毒症组治疗好转,sTREM-1、AGEs不同程度地相应下降,在脓毒症组治疗恶化,sTREM-1、AGEs不同程度地相应升高,两组间差异有统计学意义(P<0.05);脓毒症患者sTREM-1、AGEs表达水平与SOFA评分呈正相关(P<0.05)。结论:sTREM-1、AGEs的表达水平与脓毒症疾病严重程度呈正相关,其可作为预测脓毒症病情严重程度及临床预后的实验室指标。Objective:To investigate the role of sTREM-1 and AGEs in evaluating the severity and prognosis of sepsis.Method:Fifty-six patients with sepsis and 56 patients with general infection were selected.The expression levels of sTREM-1 and AGEs were detected at the day of admission,and the expression levels of sTREM-1 and AGEs were repeated at 3 and 6 days after admission.Meanwhile,the SOFA score and 20-day survival of patients were recorded.Result:sTREM-1 and AGEs in sepsis group were significantly higher than those in general infection group(P<0.05);In sepsis group,sTREM-1 and AGEs decreased in varying degrees,and sTREM-1 and AGEs increased correspondingly in varying degrees when treatment deteriorated.With significant difference between the two groups(P<0.05)in sepsis patients.There were positive correlation between the levels of sTREM-1 and AGEs.Conclusion:The expression levels of sTREM-1 and AGEs are positively correlated with the severity of sepsis,which can be used as laboratory indicators to predict the severity and clinical prognosis of sepsis.
