摘 要:目的观察比较自体肌腱、新鲜冷冻异体肌腱和冻干异体肌腱重建后交叉韧带的临床疗效。方法回顾性分析行关节镜下后交叉韧带重建术患者68例,按移植物材料不同分为自体肌腱组、新鲜冷冻肌腱组和冻干肌腱组。对所有患者最后一次随访时的膝关节各项功能评分进行比较,评价膝关节功能,并比较三组并发症发生率。用X线测量胫骨平台后移的距离评价膝关节稳定性。结果三组术后的Lysholm评分、IKDC评分和Tegner评分较术前均明显改善,P均<0.05;术后三组患者膝关节各项功能评分无明显差异,P>0.05。三组胫骨后移较术前均明显改善,P<0.05;与自体肌腱组比较,新鲜冷冻肌腱组和冻干肌腱组胫骨后移较大,P<0.05;新鲜冷冻肌腱组和冻干肌腱组之间比较,胫骨后移无明显差别,P>0.05。三组并发症的发生率无明显差异,P>0.05。结论自体和异体肌腱重建后交叉韧带均可以改善膝关节功能、恢复膝关节后方的稳定性,但在恢复膝关节后方的稳定性方面,自体肌腱优势更明显。Objective To compare the clinical efficacy of autogenous tendon,fresh frozen allogeneic tendon and freeze-dried allogeneic tendon in reconstruction of the posterior cruciate ligament of knee.Methods 68 patients undergoing arthroscopic posterior cruciate ligament reconstruction in our hospital were divided into autogenous tendon group,fresh frozen allogeneic tendon group and freeze-dried allogeneic tendon group.At the final follow up,the knee funtion scores of all patients were recorded to evaluate knee function and the backwards distance of tibial plateau was measured by X ray to evaluate knee stability.Results The Lysholm score,IKDC score and Tegner score of the three groups at final follow-up were all higher than those before operation(P<0.05),but there were no significant difference among the three groups at the final follow-up(P>0.05).The backwards distance of tibial plateau of the three groups were all larger than those before operation.Compared to autogenous tendon group,the backwards distance of tibial plateau of fresh frozen allogeneic tendon group and freeze-dried allogeneic tendon group were larger(P<0.05),but there was no significant difference between fresh frozen allogeneic tendon group and freeze-dried allogeneic tendon group(P>0.05).Conclusion Both autogenous tendon and allogeneic tendon can improve knee function and restore knee stability,but autogenous tendon is better in restoring the stability of posterior knee joint.
