KB角和ANB角评价上下颌骨前后向位置关系的指标特异度和灵敏度对比研究

摘　　要：目的建立山东地区成人正常[牙合]KB角的X线头影测量正常值范围,并与ANB角进行有关指标特异度和灵敏度方面的对比评价分析。方法按照同一标准严格筛选出山东地区成人正常[牙合]123名,男性60名,女性63名,年龄19~26岁。拍摄X线头颅定位侧位片。测量包括KB角和ANB角。计算各测量指标的均数与标准差。所有数据采用SPSS 11.0统计软件包进行统计学分析。分别以"均数±1.96标准差"和"均数±标准差"作为界值范围,筛检成人正常[牙合]样本,对比分析KB角和ANB角间指标特异度的差异;分别以"均数±1.96标准差"和"均数±标准差"作为界值范围,分别筛检成人安氏Ⅰ、Ⅱ、Ⅲ类错[牙合]畸形患者227例,对比分析KB角和ANB角间指标灵敏度的差异。结果①建立了山东地区成人正常[牙合]KB角的X线头影测量正常值范围;②KB角和ANB角在指标特异度方面无明显差异;③对于安氏Ⅱ类错[牙合]畸形,以两种界值范围进行判断,KB角的灵敏度相比ANB角都要高出22%左右;对于安氏Ⅲ类错[牙合]畸形,以"均数±标准差"作为界值范围,KB角的灵敏度相比ANB角高出18%左右。结论本研究提出了一项全新的X线头影测量指标KB角并建立了山东地区成人正常[牙合]正常值参考范围。与ANB角相比,对于安氏Ⅱ类和安氏Ⅲ类错[牙合]畸形患者,以"均数+标准差"作为界值范围时KB角的灵敏度相对更好。Objective To establish a normal reference value of the KB angle of Chinese adults with normal occlusion in Shandong province.To compare its specificity and sensitivity with ANB angle in evaluation of the anteroposterior relationship between maxilla and mandible.Methods 123 adults from Shandong province with normal occlusion were included,60 males and 63 females,age 19 to 26 years old.Lateral cephalometric radiograph of each subject was taken.KB angle and ANB angle of each subject were measured,and the means and standard deviations were calculated.SPSS Statistics 11.0 were used for analyses.Specificties of KB angle and ANB angle was compared by using mean±1.96 standard deviation and mean±standard deviation as boundary values to screen normal occlusion.Sensitivities of KB angle and ANB angle was compared by using mean±1.96 standard deviation and mean±standard deviation as boundary values to screen 227 adults of Angle Class I,Ⅱ,Ⅲmalocclusion.Results The normal reference value of KB angle of Chinese adults with normal occlusion in Shandong province was established.There was no significant difference between the specificities of KB angle andANB angle.In subjects with Angle ClassⅡmalocclusion,sensitivity of KB angle was about 22%higher than that of ANB angle by using either of the two boundary values;In subjects with Angle ClassⅢmalocclusion,sensitivity of KB angle was about 18%higher than ANB angle by using mean±standard deviation as boundary value.Conclusions The study proposes the KB angle as a completely new cephalometric measurement in evaluating anteroposterior relationship between maxilla and mandible,and the normal reference value of the KB angle of Chinese adults with normal occlusion in Shandong province is established.Compared with the ANB angle,in either Angle ClassⅡor ClassⅢmalocclusion subjects,the sensitivity of KB angle is relatively higher when using mean±standard deviation as boundary value.
