摘 要:目的探讨子痫前期孕妇脐血和胎盘中热休克蛋白90(HSP90)及缺氧诱导因子-1α(HIF-1α)的表达及意义。方法选取青岛大学医学院附属医院收治的72例子痫前期孕妇,分为子痫前期轻度组(34例)和子痫前期重度组(38例),同时选择青岛大学医学院附属医院的72例健康孕妇作为对照组。测定HSP90、HIF-1αmRNA、蛋白水平。结果子痫前期轻度组、子痫前期重度组胎盘、脐血中HSP90、HIF-1αmRNA、蛋白表达水平均高于对照组(均P<0.05);子痫前期重度组胎盘、脐血中HSP90、HIF-1αmRNA、蛋白表达水平均高于子痫前期轻度组(均P<0.05);子痫前期组HSP90、HIF-1α阳性率及表达评分高于对照组(P<0.05);子痫前期严重程度与胎盘、脐血中HSP90、HIF-1αmRNA、蛋白呈正相关(P<0.05);HSP90高表达组产后出血、胎盘早剥、弥散性血管内凝血、心衰发生率及总不良事件发生率高于HSP90低表达组(均P<0.05);HIF-1α高表达组产后出血、胎盘早剥、弥散性血管内凝血、心衰发生率及总不良事件发生率高于HIF-1α低表达组,(均P<0.05)。结论子痫前期患者胎盘、脐血组织中HSP90、HIF-1αmRNA、蛋白表达水平升高,与子痫前期的严重程度呈正相关;HSP90、HIF-1α高表达与子痫前期孕妇不良预后相关。Objective To explore the expressions and significance of heat shock protein 90(HSP90)and hypoxia-inducible factor-1α(HIF-1α)in umbilical cord blood and placenta of preeclampsia pregnant women.Methods Seventy-two preeclampsia pregnant women treated in Affiliated Hospital of Medical College of Qingdao University were selected and divided into mild preeclampsia group(34 cases)and severe preeclampsia group(38 cases),72 healthy pregnant women were selected as control group.The levels of HSP90,HIF-1αmRNA and proteins were measured.Results The levels of HSP90,HIF-1αmRNA and proteins in placenta and umbilical cord blood of mild preeclampsia group and severe preeclampsia group were statistically significantly higher than those in control group(P<0.05).The levels of HSP90,HIF-1αmRNA and proteins in placenta and umbilical cord blood of severe preeclampsia group were statistically significantly higher than those in mild preeclampsia group(P<0.05).The positive rates and scores of HSP90 and HIF-1αin preeclampsia group were statistically significantly higher than those in control group(P<0.05).The severity of preeclampsia was positively correlated with the levels of HSP90,HIF-1αmRNA and proteins in placenta and umbilical cord blood(P<0.05).The incidence rates of postpartum hemorrhage,placental abruption,DIC,and heart failure,the total incidence rate of adverse events in HSP90 high expression group were statistically significantly higher than those in those in HSP90 low expression group(P<0.05).The incidence rates of postpartum hemorrhage,placental abruption,DIC,and heart failure,the total incidence rate of adverse events in HIF-1αhigh expression group were statistically significantly higher than those in those in HIF-1αlow expression group(P<0.05).Conclusion The levels of HSP90,HIF-1αmRNA and proteins in placenta and umbilical cord blood of preeclampsia pregnant women increase,which are positively correlated with severity of preeclampsia,high expressions of HSP90 and HIF-1αare related to adverse prognosis of preeclam
