原发性高血压患者中估算肾小球滤过率水平及左心房直径与心房颤动的相关性分析

摘　　要：目的研究原发性高血压患者估算肾小球滤过率(eGFR)水平及左心房直径与心房颤动发生的相关性。方法纳入2017年11月至2018年11月间在我院住院的原发性高血压合并心房颤动患者(HPAF组)104例和原发性高血压未合并房颤患者(HP组)110例为研究对象,统计一般临床资料和入院抽血化验指标。应用二元Logistic回归分析及ROC曲线分析原发性高血压患者eGFR水平及左心房直径与心房颤动的关系。结果二元Logistic回归分析示,eGFR水平及左心房直径可作为原发性高血压患者预测心房颤动的危险因素。eGFR ROC曲线显示曲线下面积为0.72,截断点为68.12 ml-min-・1.73m 2,左心房直径ROC曲线显示曲线下面积为0.84,截断点为35.5 mmo结论eGFR水平下降与左心房扩大同样对于原发性高血压患者发生心房颤动具有预测价值。Objective To study the correlation between eGFR level,left atrial diameter and atrial fibrillation in patients with essential hypertension.Methods From November 2017 to November 2018,104 patients with hypertension complicated with atrial fibrillation(HP-AF group)and 110 patients of hypertension without atrial fibrillation(HP group)were included in the study.The general clinical data and blood test indicators were recorded.Binary Logistic regression and ROC curve were used to analyze the relationship between eGFR,left atrial diameter and atrial fibrillation in patients with essential hypertension.Results In binary Logistic regression analysis,both eGFR level and left atrial diameter can be used as risk factors for predicting atrial fibrillation in patients with essential hypertension.The ROC curve of eGFR showed that the area under the curve was 0.72 and the cut-off point was 68.12 ml•min-1•1.73m-2.The ROC curve of left atrial diameter showed that the area under the curve was 0.84 and the cut-ofF point was 35.5 mm.Conclusion Decreased eGFR level and increased left atrial diameter are both predictive factors for atrial fibrillation in patients with essential hypertension.
