摘 要:目的探讨高压氧舱治疗急性高原性肺水肿(high altitude pulmonary edema,HAPE)的临床疗效及安全性。方法采用回顾性队列研究方法,收集西藏阿里地区人民医院2017年03月~2017年12月收治的102例急性高原性肺水肿患者,随机分为常规治疗组、高压氧舱治疗1组、高压氧舱治疗2组,每组各34例。常规治疗组予以常规治疗,高压氧舱治疗1组为常规治疗基础上做高压氧舱次数<3次,高压氧舱治疗2组为常规治疗基础上做高压氧舱次数3次,对3组的临床疗效及安全性进行对比观察。结果治疗后常规治疗组与高压氧舱治疗1组对比呼吸频率、心率、PaO2差异均有统计学意义(P<0.05),PaCO2差异无统计学意义(P>0.05),常规治疗组与高压氧舱治疗2组对比呼吸频率、心率、PaCO2、PaO2差异均有统计学意义(P<0.05),高压氧舱治疗1组与高压氧舱治疗2组对比心率、PaO2差异有统计学意义(P<0.05)。总住院天数的优势高压氧舱治疗2组>高压氧舱治疗1组>常规治疗组(P<0.05),总有效率高压氧舱治疗2组>高压氧舱治疗1组>常规治疗组(P<0.05);3组患者均未出现明显不良反应。结论常规治疗联合高压氧舱(每日3次)对急性高原性肺水肿的临床疗效显著,是一种安全有效的治疗方法,可供临床借鉴。Objective To explore the clinical efficacy and safety of hyperbaric oxygen chamber in the treatment of high altitude pulmonary edema. Methods A retrospective cohort study was used to collect 102 patients with acute high altitude pulmonary edema from March 2017 to December 2017 in Ali District People’s Hospital of Tibet. They were randomly divided into routine treatment group, hyperbaric oxygen chamber treatment group 1 and hyperbaric oxygen chamber treatment group 2, with 34 cases in each group. The routine treatment group was given routine treatment, while the hyperbaric oxygen cabin treatment group 1 was given hyperbaric oxygen cabin less than 3 times on the basis of routine treatment, and the hyperbaric oxygen cabin treatment group 2 was given hyperbaric oxygen cabin more than 3 times on the basis of routine treatment. The clinical efficacy and safety of the three groups were compared. Results There were significant differences in respiratory rate, heart rate and PaO 2 between the routine treatment group and the hyperbaric oxygen chamber treatment group 1(P<0.05), but no significant differences in PaCO 2(P>0.05). There were significant differences in respiratory rate, heart rate, PaCO 2 and PaO 2 between routine treatment group and hyperbaric oxygen chamber treatment group(P<0.05). There were significant differences in heart rate and PaO 2 between group 1 and group 2(P<0.05). The total effective rate of hyperbaric oxygen treatment group 2 > hyperbaric oxygen treatment group 1 > conventional treatment group(P < 0.05), the total effective rate of hyperbaric oxygen treatment group 2 > hyperbaric oxygen treatment group 1 > conventional treatment group(P<0.05). No significant adverse reactions were found in all the three groups. Conclusion Routine treatment combined with hyperbaric oxygen chamber has a significant clinical effect on acute high altitude pulmonary edema. It is a safe and effective treatment method.
