摘 要:目的探讨儿童早期预警评分(PEWS)对急诊观察室患儿病情评估的价值。方法选取本院2016年6月~2017年6月急诊观察室就诊的250例患儿作为研究对象,对其进行PEWS评分,将72小时内有无抢救或住院作为评估标准,分析PEWS评分系统对急诊观察室患儿病情的评估价值。结果 250例患儿中,死亡1例(0.40%),其PEWS评分为12分,存活患儿平均评分为(2.32±0.71)分;住院99例(39.60%),其中留观时PEWS评分为8~10分29例(29.29%),评分为5~7分50例(50.51%),评分低于5分13例(13.13%)。入住儿科重症监护病房(PICU)抢救患儿7例(包括1例死亡患儿),PEWS评分均高于10分,显著高于非入住PICU的患儿(P<0.05);留观24小时后带药回家的患儿151例(60.40%),PEWS评分均低于5分,平均(2.24±0.72)分,显著低于住院患儿的评分(t=6.57,P<0.01)。结论 PEWS评分系统可作为快速、有效评估急诊观察室患儿病情的重要方法,其方法简便可行、操作性强,有着良好的分辨能力,可在临床应用推广。Objective To explore the value of PEWS in the evaluation of children’s condition in emergency observation room. Methods From June 2016 to June 2017, 250 children in the emergency observation room of our hospital were selected as the study objects to score their PEWS. Taking whether there is rescue or hospitalization within 72 hours as the evaluation standard, the evaluation value of PEWS scoring system on the condition of children in emergency observation room was analyzed. Results Among the 250 children, 1 died(0.40%), with a PEWS score of 12 and an average score of 2.32±0.71. 99 patients(39.60%) were hospitalized, 29 patients(29.29%) scored 8-10. When they stayed in the hospital, 50 patients(50.51%) scored 5-7 and 13 patients(13.13%) scored lower than 5. 7 children(including one dead child) were rescued in the pediatric intensive care unit(PICU). The scores of PEWS were all higher than 10, significantly higher than those of non PICU children(P<0.05). 151 children(60.40%) returned home with drugs 24 hours later. The scores of PEWS were lower than 5(2.24±0.72) and significantly lower than those of hospitalized children(t=6.57, P<0.01).Conclusion The PEWS scoring system can be used as an important method for rapid and effective evaluation of children’s condition in the emergency observation room. The method is simple and feasible, with strong operability and good resolution, and can be applied in clinical practice.
