补骨汤治疗骨质疏松性椎体骨折骨不愈合临床研究

摘　　要：目的:观察补骨汤治疗骨质疏松性椎体骨折(OVFs)骨不愈合的临床疗效。方法:将我院自2014年1月~2017年5月间收治的89例OVFs骨不愈合患者纳入研究范围,按区组随机化分为观察组(n=46)与对照组(n=43),对照组给予抗骨质疏松干预,观察组在此基础联合补骨汤;对比两组临床疗效、影像学指标、骨密度及骨代谢指标,并对胸腰背痛、脊柱功能进行评分。结果:治疗后观察组疗效等级优于对照组;同时,观察组治疗前后椎体前缘及中间高度、Cobb角差值低于对照组(P<0.05);BMD上升并高于对照组,CTX-Ⅰ、TRACP5b下降并低于对照组;胸腰背疼痛及ODI脊柱评分均下降亦低于对照组(P<0.05)。结论补骨汤治疗OVFs骨不愈合疗效佳,于维持稳定的椎体生物学高度、改善骨密度、骨代谢、胸腰背疼痛及脊柱功能均有重要意义。Objective:To study the clinical curative effect of using Bugu Decoction in the treatment of bone nonunion of osteoporotic vertebral fractures(OVFs).Methods:A total of 89patients with bone nonunion of OVFs who were admitted to the hospital from January 2014to May 2017were included in the study.They were randomly divided into observation group(n=46)and controlled group(n=43).The controlled group was given anti-osteoporosisintervention,while the observation group was additionally treated with Bugu Decoction.The clinical curative effect,imaging indexes,bone mineral density and bone metabolism indexes were compared between the two groups.The chest and low back pain and spinal functionwere scored.Results:The curative effect of observation group was better than that of the controlled group.The difference values of fanterior and middle vertebral body height and Cobb angle of the observation group before and after treatment were lower than those of the controlled group(P<0.05).The BMD was higher,while CTX-I and TRACP5b were lower in the observation group than in the controlled group.Besides,chest and low back pain and ODI spinal scores of the observation group were lower than those of the controlled group(P<0.05).Conclusion:Bugu Decoction is effective in treating bone nonunion of OVFs.It is important for maintaining stable vertebral height,improving bone mineral density and bone metabolism,relieving chest and lowback pain and improving spinal function.
