摘 要:目的探讨北京市朝阳区汽感样病例(ILI)中的肠道病毒(EV)病原谱构成及分子流行病学特征,为流感防控提供理论依据。方法采集2016年1月-2018年12月,北京中日友好医院、首都医科大学附属北京朝阳医院2家哨点医院就诊的6327例ILI咽拭子标本,采用RT-PCT检测常见的11种呼吸道病原,对EV阳性标本采用普通PCR扩增病毒5-UTR片段,测序并进行基因分型。结果6327例ILI中,共检出EV阳性标本75例(1.19%),各年不同月份间及3年合计不同年龄组间EV阳性率比较差异有统计学意义(P均<0.05)。75例EV阳性标本其中28例分型成功,包括10例CV-A6.7例EV-68、6例CV-A10、2例CV-B1及CV-A2、CV-A4和CV-A5各1例。7例EV-68阳性标本进行聚类分析,5例A型,2例B型。结论2016-2018年北京市朝阳区ILI中EV病原谱主要包括CV-A6、EV-68、CV-A10、CV-B1、CV-A2、CV-A4和CV-A5,其中CV-A6、EV-68和CV-A10为优势病毒型,其中EV-68存在2个型别的共循环。Objective To investigate the pathogen sectrum and molecular epidemiological characteristcs of enterovir-us(EV)in cases of influenza-like iIness(ILI)in Chaoyang District,Beijing.Methods A total of 6,327 throat swabs from patients with ILI were collected at 2 sentinel hospitals(the Chaoyang Hospital of Capital Medical University and the China-Japan Friendship Hospital of the Ministry of Health)in Beijing from January 2016 to December 2018.Fluorescence quantitative RT-PCT was used to detect 11 respiratory viruses.EV-positive specimens were identified,and the 5-UTR fragment of EV was amplified using PCR,sequenced,and genotyped.Data were presented in tables and figures and the Chi-square test or Fisher's exact test used to compare between-group differences in percentages.Associations were consid-ered significant if the p-value was less than 5%.The data were analyzed using the software SPSS version 20.0.Results Of the 6,327 outpatients who sought treatment from 2016 to 2018,1,773 outpatients tested positive for a pathogen.Results indicated that 11 respiratory pathogens circulated in Bejing,and the most frequently detected pathogens were hu-man influenza virus(FluA/B)(16.15%;1,022/6,327),human rhinovirus(hRV)(3.38%;214/6,327),and mycoplas-ma(MP)(2.01%;127/6,327).Positivity for EV differed significantly by season and by age group during the 3 years studied(P<0.05).A total of 75(1.19%)EV-positive specimens were detected,28 of which were successfully classi-fied.CV-A6 was detected in 10 patients,EV-68 was detected in 7,CV-A10 was detected in 6,CV-B1 was detected in 2,CV-A2 was detected in 1,CV-A4 was detected in 1,and CV-A5 was detected in 1.A cluster analysis of 7 EV-68 positive samples indicated that type A was present in 5 and type B was present in 2.Conclusion The pathogen spectrum of EV in ILI in the Chaoyang District of Beijing from 2016-2018 included CV-A6,EV-68,CV-A10,CV-B1,CV-A2,CV-A4,and CV-A5.The dominant viruses were CV-A6,EV-68,and CV-A10.Two types of EV-68 were co-circulating in this region.
