摘  要:目的观察人参总皂苷和异氟醚联合预处理对心肌缺血再灌注损伤大鼠的作用,并从TLR-4/NF-κB信号通路探讨其发生机制。方法选取雄性清洁级SD大鼠50只,分为假手术组、灌注损伤组、异氟醚预处理组、人参总皂苷组和异氟醚+人参总皂苷预处理组,每组各10只。对应手术处理后处死,收集血液测定炎性因子IL-6及TNF-α的含量,收集心脏计算心肌梗死面积,观察心肌组织病理学,检测NF-κB和TLR4蛋白和mRNA的表达特点。结果假手术组心肌细胞呈纤维状排列,心肌缺血再灌注损伤组有明显细胞形态紊乱,与心肌缺血再灌注损伤组相比,其余各组均有一定程度的减轻。与假手术组相比,心肌缺血再灌注损伤组心肌NF-κB、心肌TLR4、血清IL-6和TNF-α明显增加(P<0.05)。与心肌缺血再灌注损伤组相比,异氟醚组、人参总皂苷组和异氟醚+人参总皂苷组心肌梗死面积、IL-6、TNF-α、心肌NF-κB和TLR4表达均明显降低(P<0.05)。此外,异氟醚+人参总皂苷组心肌梗死面积、血清IL-6、血清TNF-α、心肌NF-κB和TLR4表达低于异氟醚组和人参总皂苷组(P<0.05)。结论人参总皂苷和异氟醚联合预处理能有效改善心脏病理学和心肌梗死面积,其作用机理可能是二者联合应用降低了心肌中TLR4和NF-κB蛋白及mRNA表达,进而改善炎症反应有关。Objective To observe the effect of ginsenoside and isoflurane combined pretreatment on myocardial ischemia-reperfusion injury in rats,and to explore the mechanism of TLR 4/NFκB signaling pathway.Methods 50 male SD rats were divided into sham operation group,perfusion injury group,isoflurane pretreatment group,ginsenoside group and isoflurane+ginsenoside pretreatment group.After corresponding operation,the rat were killed,the contents of IL-6 and TNF-α in blood were detected,the infarct area was calculated,the myocardial histopathology was observed,and the expression characteristics of NF-κB and TLR4 Protein and mRNA were detected.Results The myocardial cells in the sham operation group were arranged in a fibrous pattern,and the cells in the ischemia-reperfusion injury group were obviously disordered.Compared with sham operation group,NF-κB,TLR4,IL-6 and TNF-α in myocardial ischemia-reperfusion injury group increased significantly(P<0.05).Compared with myocardial ischemia-reperfusion injury group,the expression of myocardial infarction area,IL-6,TNF-α,NF KB and TLR4 in isoflurane group,ginsenoside group and isoflurane+ginsenoside group decreased significantly(P<0.05).In addition,the expression of myocardial infarction area,serum IL-6,serum TNF-α,myocardial NF-κB and TLR4 in isoflurane+ginsenoside group were lower than those in isoflurane group and ginsenoside group(P<0.05).Conclusion The combined pretreatment of ginsenoside and isoflurane can effectively improve the cardiac pathology and myocardial infarction area.The mechanism may be related to reduce the expression of TLR4 and NF-κB protein and mRNA in myocardium,and improve the inflammatory response.
