摘  要:目的评估血液微小RNA-1(miRNA-1)对东亚人群早期急性心肌梗死(AMI)的诊断价值。方法通过检索PubMed、Embase、Cochrane数据库、中国知网(CNKI)、中国生物医学文献数据库(CBM)、维普网(VIP)和万方数据知识服务平台等数据库建库截至2018年10月的相关文献。以诊断试验准确性研究质量评价量表(QUADAS-2)对文献进行质量评价。采用合并敏感度(SEN)、特异度(SPE)、阳性似然比(PLR)、阴性似然比(NLR)、诊断优势比(DOR)和综合受试者曲线下面积(AUC)评估miRNA-1对东亚人群早期AMI的总体检测性能。采用RevMan5.3以及Stata12.0软件进行所有的统计分析。结果共纳入14篇符合要求的研究文献,miRNA-1对AMI的诊断总体汇总数据如下:灵敏度为0.79,特异度为0.89,阳性似然比为7.39,阴性似然比为0.23,诊断优势比为31.56,综合受试者曲线下面积为0.90。结论血液miRNA-1水平能作为东亚人群早期AMI诊断生物标志物。Objective To evaluate the diagnostic value of blood microRNA-1(miRNA-1)for East Asian early acute myocardial infarction(AMI).Methods The relevant literatures of the PubMed,Embase,Cochrane database,China National Knowledge Infrastructure(CNKI),China Biomedical Literature Database,CQVIP and Wanfang Data Knowledge Service Platform were built as of October 2018.The quality of the literature was evaluated by the Diagnostic Test Accuracy Study Quality Evaluation Scale(QUADAS-2).miRNA-1 assessment was assessed by combined sensitivity(SEN),specificity(SPE),positive likelihood ratio(PLR),negative likelihood ratio(NLR),diagnostic odds ratio(DOR),and comprehensive subject area under the curve(AUC)Overall detection performance of East Asian early AMI.All statistical analyses were performed using RevMan 5.3 and Stata 12.0 software.Results 14 eligible studies were included.The overall data of miRNA-1 diagnosis of AMI included sensitivity(0.79),specificity(0.89),positive likelihood ratio(7.39),negative likelihood ratio(0.23)and the composite subject curve(0.90).Conclusion Blood miRNA-1 level can be used as a biomarker for early diagnosis of AMI in East Asian population.
