运用营卫气血理论辨治冠状动脉重度狭窄

摘　　要：冠状动脉重度狭窄是冠心病的严重血管病变,主要以经皮冠状动脉介入治疗或外科手术治疗为主。基于营卫气血理论认识冠状动脉狭窄进展规律,提出冠状动脉重度狭窄属营卫失调、毒瘀阻络病变,治疗应从气血的运行与生成着手,调和营卫,维持气血运行,调和脉管内外稳态;益肾健脾,调节气血生成,平衡机体阴阳气血。祛痰逐瘀应贯穿治疗始终,立足斑块进展规律,先以活血解毒法阻抑斑块进展,后以软坚散结法消散斑块实邪。Severe coronary artery stenosis is a serious vascular disease of coronary heart disease.The treatment is mainly based on percutaneous coronary intervention or surgical treatment.Based on the theory of nutrient qi,defensive qi,qi and blood,the progression rule of coronary stenosis is understood.It is proposed that severe coronary stenosis belongs to disorder of nutrient qi and defensive qi and toxin and stasis blocking the collaterals.The treatment should start from the movement and generation of qi and blood,regulate nutrient qi and defensive qi,maintain the movement of qi and blood,regulate the steady state inside and outside the vessels.Tonifying the kidney and invigorating the spleen is performed to regulate the generation of qi and blood,and balance the yin,yang,qi and blood of body.Eliminating the phlegm and resolving the stasis is running through the whole treatment.Base on the regularity of plaque progression,the method of activating blood circulation and detoxification is firstly used to inhibit the plaque progression,and then the method of softening and resolving hard mass is used to dissipate the excess pathogen of plaque.
