经皮肾镜碎石术后全身炎症反应综合征的危险因素分析

摘　　要：目的分析经皮肾镜碎石术(PCNL)后全身炎症反应综合征的危险因素。方法回顾性分析2016年6月~2018年12月于我院行PCNL的316例患者的临床资料,分析术后全身炎症反应综合征的危险因素。结果PCNL术后,患者全身炎症反应综合征的发生率为17.72%。单因素分析结果显示,不同结石成分、术前尿白细胞、术前尿培养、手术时间、糖尿病病史的患者术后全身炎症反应综合征发生率比较,差异有统计学意义(P<0.05)。多因素Logistic分析结果显示,结石成分(β=0.885,SE=1.356,OR=2.05,95%CI=1.97~3.78,P<0.05)、术前尿白细胞阳性(β=0.654,SE=1.297,OR=1.11,95%CI=1.02~1.22,P<0.05)、术前尿培养阳性(β=0.698,SE=0.958,OR=2.30,95%CI=1.96~2.86,P<0.05)、手术时间≥60 min(β=0.786,SE=0.958,OR=1.08,95%CI=1.01~1.19,P<0.05)、糖尿病病史(β=0.958,SE=1.205,OR=1.09,95%CI=1.02~1.14,P<0.05)均是PCNL术后全身炎症反应综合征的独立危险因素(P<0.05)。结论PCNL术后全身炎症反应综合征的独立危险因素为:结石成分、术前尿白细胞阳性、手术前尿培养阳性、手术时间≥60 min以及糖尿病病史。Objective To investigate the risk factors of systemic inflammatory response syndrome after percutaneous nephrolithotomy(PCNL).Methods The clinical data of 316 patients undergoing PCNL in our hospital from June 2016 to December 2018 were retrospectively analyzed,and the risk factors of systemic inflammatory response syndrome after operation were analyzed.Results After PCNL,the incidence of systemic inflammatory response syndrome was 17.72%.Univariate analysis showed that the incidence of systemic inflammatory response syndrome was different among patients with different stone components,preoperative urine leukocytes,preoperative urine culture,operation time and diabetes history(P<0.05).Multivariate Logistic analysis showed that stone components(β=0.885,SE=1.356,OR=2.05,95%CI=1.97-3.78,P<0.05),preoperative urine leukocytes positive(β=0.654,SE=1.297,OR=1.11,95%CI=1.02-1.22,P<0.05),preoperative urine culture positive(β=0.698,SE=0.958,OR=2.30,95%CI=1.96-2.86,P<0.05),operation time≥60 min(β=0.786,SE=0.958,OR=1.08,95%CI=1.01-1.19,P<0.05),history of diabetes mellitus(β=0.958,SE=1.205,OR=1.09,95%CI=1.02-1.14,P<0.05)were independent risks of systemic inflammatory response syndrome after PCNL.Conclusion The independent risk factors of systemic inflammatory response syndrome after PCNL are:stone composition,urinary leukocytes positive before operation,positive urine culture before operation,operation time≥60 min and diabetes history.
