加速康复外科理念在糖尿病患者足部创面皮瓣修复术围术期的应用效果

摘　　要：目的探讨加速康复外科理念(ERAS)在带蒂皮瓣修复糖尿病足(DF)创面中的应用效果。方法选取2017年1月~2019年1月我院收治的60例DF患者作为研究对象,按照随机数字表法分为对照组(30例)和试验组(30例)。对照组用常规围术期护理,试验组采用ERAS围术期护理,比较两组并发症总发生率、术后疼痛评分、血糖水平、排便时间、住院时间、皮瓣存活率。结果试验组并发症总发生率、术后疼痛评分和血糖水平低于对照组,排便时间和住院时间短于对照组,皮瓣存活率高于对照组,差异均有统计学意义(P<0.05)。结论在DF创面皮瓣修复术中用ERAS围术期管理,可有效减少并发症发生,提高皮瓣存活率,促进患者术后快速康复。Objective To explore the effect of application effect of the accelerated rehabilitation surgery(ERAS)in repairing foot wounds of diabetic(DF)patients with pedicled flap.Methods From January 2017 to January 2019,60 DF patients admitted to our hospital were selected and divided into the control group(n=30)and the experimental group(n=30)according to the random number table method.The control group used the conventional measures while the experimental group used the measures of the ERAS during the perioperative period.The indexes such as the total rate of complications,postoperative pain score,blood glucose level,defecation time,hospital stay and flap survival rate between the two groups were compared between two groups.Results The total rate of complications,postoperative pain score and blood glucose level were lower in the experimental group than those in the control group,and the defecation time and hospital stay were shorter than those in the control group,and the flap survival rate was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusion In the treatment of DF patients with pedicled flaps,the application of accelerated rehabilitation surgery ERAS perioperative management can effectively reduce the occurrence of complications,improve the flap survival rate and promote rapid recovery after surgery.
