摘　　要：目的通过药师对某院处方质量管理前置审核的效果分析,探讨处方管理前置审核对合理用药管理模式合理发展的影响。方法调取某院2018年5月1日-2018年5月31日处方前置审核中干预的486张处方为干预前组与2019年5月1日-2019年5月31日处方前置审核干预的2451张处方为干预后组,根据对照组干预失败、双签字等问题分析原因制定优化方案,通过更新软件架构,建立医药沟通机制,比较干预前后两样本问题的变化。结果干预前组干预处方并成功数量为328份,干预后组为1844份;干预失败占比显著降低,由干预前的9.88%下降至干预后的3.96%,双签字问题占比由干预前的22.63%降到干预后的20.81%。结论通过优化审核软件、建立与临床医师的有效沟通渠道等方式,可以促进了处方前置审核工作的合理化发展,优化就医、取药环节,有效地发挥药师在安全合理用药中的作用。Objective To accumulate and learn the experience of successful and unsuccessful intervention, improve the ability of pharmacists in pre-prescription auditing, and promote the healthy and rational development of pre-prescription auditing.Methods 486 prescriptions intervened in the pre-prescription review of a hospital from May 1, 2018 to May 31, 2018 were collected and compared with 2451 prescription data of pre-prescription audit intervention from May 1, 2019 to May 31, 2019. Based on the analysis of the reasons for failure of intervention and double signatures, the optimization scheme was formulated, and the software architecture was updated to establish the pre-prescription audit intervention. Medical communication mechanism was used to compare the changes of intervention before and after the two samples.Results Chi-square test was used to compare the data of the two groups, P<0.01 before and after intervention, the difference was statistically significant. The number of intervention prescriptions and intervention success increased significantly(328 vs. 1844), the proportion of intervention failure decreased significantly(9.88% vs. 3.96%) and the proportion of double signatures decreased slightly(22.63% vs. 20.81%).Conclusion By optimizing the auditing software and establishing effective communication channels with clinicians, the rationalization of the pre-prescription auditing work in a hospital is promoted, and the links of seeking medical treatment and taking medicine are more optimized, thus fully and effectively playing the role of pharmacists in ensuring the safety and rationality of drug use.
