延续护理在缺血性脑卒中后遗症患者居家照护的应用效果

摘　　要：目的:探讨延续护理在缺血性脑卒中患者居家护理的应用效果。方法:将90例缺血性脑卒中患者根据自理能力分层,每层患者随机分为干预组和对照组。干预组实施延续护理,对照组实施常规护理。比较两组患者出院时和1个月后生活质量评分,两组照顾者出院时和1个月后照顾负担得分。结果:第1次评估3个等级患者的生活质量,干预组和对照组的差异均无统计学意义(P>0.05),第2次评估在轻度依赖组躯体功能和重度依赖患者心理功能有统计学差异(P<0.05).中度和重度依赖患者干预组照顾者照顾负担与对照组2次评估的差异均有统计学意义(P<0.05)。结论:延续护理方案能更好地帮助缺血性脑卒中患者从医院过渡到社区和家庭。Objective:To discuss the effect of continuous nursing on home care for patients with ischemic stroke sequelae.Methods:90 patients with cerebral ischemic stroke were stratified according to their self-care abilities,and randomized into the intervention group and control group.Patients in the intervention group implemented continuous care,while patients in the control group underwent routine care.The quality of life scores of the two groups of patients at discharge and 1 month after being discharged from hospial were compared,and the caregiver burden scores at the time of the patients*discharge and 1 month after the patients*being discharged from hospital were compared too.Results:The quality of life of patients with 3 grades was evaluated at the first time,there were no significant differences in the patients*quality of life between the intervention group and the control group(P>0.05).At the second assessment,there were statistically significant differences in physical functions between the mildly dependent group and the severely dependent group(P<0.05).There was statistical difference of caregiver burden scores between the moderate and severely dependent patients in the intervention group,and patients in the control group as well(P<0.05).Conclusion:Continuous nursing programs can better help patients with cerebral ischemic stroke sequelae transit from hospital nursing to community and home care.
