高效联合抗反转录病毒治疗艾滋病病人生存质量及其影响因素分析

摘　　要：目的了解甘肃省接受高效联合抗反转录病毒治疗(HAART)艾滋病病人(HIV/AIDS)的生存质量,探讨相关影响因素,为提高随访管理和改善病人生存质量提供参考。方法收集2018年现住址为甘肃省6个市正在接受HAART治疗的332例HIV/AIDS一般人口学信息,采用MOS-HIV量表评价全部研究对象生存质量,采用t检验和方差分析比较均数差异。结果HAART治疗HIV/AIDS总体健康感、心理健康和生理健康最终评分地区差异有统计学意义(F=10.760,P<0.01),大专及以上文化程度者的总体健康感(t=2.642)、生理健康(t=2.784)评分较高中及以下文化程度者高(P<0.01),35岁以下者的总体健康感(t=2.562)、心理健康(t=2.273)评分高于35岁及以上者(P<0.05),HIV期总体健康感高于艾滋病期(t=2.601,P<0.05),治疗后的生存质量高于治疗前(P<0.01),CD4水平350/μl及以上者总体健康感(t=3.210)和心理健康(t=3.254)最终评分均较350/μl以下者高(P<0.01)。结论HAART治疗明显提高了HIV/AIDS的生存质量;提高随访管理质量、加强心理疏导以及救助及关怀措施落实,可提高HIV/AIDS生存质量。Objective To understand the life quality of HIV/AIDS treated with HAART in Gansu province,explore the influencing factors,in order to provide basic data for improving follow-up management and life quality of the patients.Methods The demographic information of the 332 HIV/AIDS receiving HAART in Gansu province was collected and their life quality was evaluated by MOS-HIV,so as to analyze and discover the factors affecting life quality of HIV/AIDS.Analysis of variance and t-test was taken to compare differences.Results There were statistical differences in general health(GH),mental health score(MHS)and physiological health score(PHS)of HIV/AID(F=10.760,P<0.01).The GH(t=2.642)and PHS(t=2.784)of those with more education background was higher than that of the others(P<0.01),and the GH(t=2.562)and MHS(t=2.273)of those under 35 year-old were higher than that of those over 35 year-old(P<0.05).The GH of HIV was higher than that of AIDS(t=2.601,P<0.05).The life quality was higher after HAART than before HAART(P<0.01).The GH(t=3.120)and MHS(t=3.254)were higher in patients with CD4 over 350/μl than those with CD4 under 350/μl(P<0.01).Conclusions HARRT can improve life quality of HIV/AIDS significantly.Improving follow-up quality,strengthening psychological counseling and implementing rescue and care measures can improve the patients’quality of life.
