摘　　要：目的探讨利用磁共振T 2 mapping成像评估干燥综合征(sjogren’s syndrome,SS)早期腮腺损伤的可行性。方法前瞻性纳入69例SS患者与39例志愿者,分别对患者和志愿者二组行T 1W、T 2-STIR、T 2 mapping扫描,依据Makula分级标准将SS患者分为0~3级,测量二组腮腺T 2值。独立样本t检验用于比较两组差异,采用受试者工作特征曲线(receiver operating characteristic,ROC)分析T 2值在诊断SS腮腺腺体早期损伤时的准确性,Spearman秩和检验分析T 2值和形态学分级的相关性。结果SS全部分级患者平均T 2值高于健康志愿者(70.27±3.61)ms vs(67.16±3.97)ms,P<0.01,且SS 0级患者高于健康志愿者(71.25±3.39)ms vs(67.16±3.97)ms,P<0.01。ROC分析示T 2值诊断SS全部分级患者与0级患者的曲线下面积(area under the ROC curve,AUC)为0.756、0.825,准确性为73.14%、76.80%。T 2值与SS腮腺损伤分级具有相关性(r=-0.360,P<0.01)。结论T 2 mapping能够客观、无创性的评估SS患者腮腺的早期损伤,为SS早期诊断提供一定的临床诊断价值。Objective To evaluate the feasibility of the early damage of parotid glands in the sjogren's syndrome(SS)with magnetic resonance T 2 mapping imaging.Methods 69 SS patients and 39 volunteers were enrolled prospectively,two groups of patients and volunteers were scanned with T 1W,T 2-STIR and T 2 mapping,respectively.SS patients were divided into grade 0~3 grades according to the Makula grade,and T 2 values of two groups were measured.Independent sample t test was used to compare the differences between the two groups.The receiver operating characteristic(ROC)curve was used to analyze the accuracy of T 2 values in the diagnosis of early damage of parotid glands.Spearman test was used to analyze the correlation between T 2 values and morphological grade.Results The mean T 2 values of parotid glands of all SS patients was higher than that of healthy volunteers,(70.27±3.61)ms vs(67.16±3.97)ms,P<0.01 and SS grade 0 was also higher than that of healthy volunteers(71.25±3.39)ms vs(67.16±3.97)ms,P<0.01.The ROC analysis showed that the area under the ROC curve(AUC)in diagnosing all grade patients and grade 0 patients was 0.756,0.825,and the accuracy was 73.14%,76.80%(all P<0.01).T 2 values were significantly correlated with the grade of parotid glands damage(r=-0.360,P<0.01).Conclusion T 2 mapping can objectively and noninvasively evaluate the early damage of the parotid glands in SS patients,providing certain clinical value for the early diagnosis of SS.
