摘 要:目的探讨在保证临床诊断前提下,优化儿童胸部数字化摄影(DR)曝光参数,降低患儿受照剂量。方法将就诊400例患儿按照年龄分成三个区组,随机分配到4个照射条件组(60kV组;70kV组;80kV组;90kV组)。依儿童胸廓大小严格限制照射野,源像距离(SID)为1m,小焦点,自动曝光控制(AEC)模式中野电离室曝光。曝光结束后,记录曝光量(mAs)、剂量面积乘积(DAP)值和曝光时间(ms)。图像传入PACS采取双盲法评价图像质量。结果1)不同千伏下比较F值发现四组图像质量差异无显著性(P>0.05);2)四组数据中mAs和DAP分别为:(3.48±2.12)mAs、(0.81±0.55)dGycm 2;(1.53±0.71)mAs、(0.51±0.26)dGycm 2;(0.62±0.20)mAs、(0.41±0.16)dGycm 2;(0.50±0.19)mAs、(0.35±0.26)dGycm 2。不同千伏下mAs和DAP差异有显著性(P<0.05)。结论1岁以内婴儿建议使用次高千伏(70~80kV)摄影,1岁以上患儿建议使用高千伏(80~90kV)摄影,可降低患儿受照剂量。Objective To optimize the exposure parameters of chest DR radiography and to reduce the radiation dose in children under the premise of diagnosis.Methods 400 hospitalized children were divided into three groups according to their ages and,subsequently,randomly distributed to four groups with different exposure conditions i.e.,60 kV group,70 kV group.80 kV group,and 90 kV group.In accordance with the children’s thoracic size,the radiation field was strictly limited.The source as distance(SID)was 1 m,with small focus and the infield ionization chamber with automatic exposure control(AEC)mode exposure.After the exposure,the exposure amount(mAs),dose area product(DAP)and exposure time were recorded.Image quality was evaluated by double-blind method in PACS.Results 1)Fvalues at different kilovolts showed no significant difference in image quality among the four groups(P>0.05);2)The mAs and DAP in four groups were(3.48±2.12)mAs,(0.81±0.55)dGycm 2;(1.53±0.71)mAs,(0.51±0.26)dGycm 2;(0.62±0.20)mAs,(0.41±0.16)dGycm 2;and(0.50±0.19)mAs,(0.35±0.26)dGycm 2,respectively.The mAs and DAP exhibited significant differences under the conditions of different kilovolts(P<0.05).Conclusion Four common photographic conditions are compared to ensure the imaging quality.The second-highest kV(70~80 kV)photography is recommended for infants under age of 1 year old,while,high-kV(80~90 kV)photography is recommended for children over the age of 1 year old,which can effectively reduce the radiation dose.
