局部皮瓣修复肿瘤切除术后头面部皮肤软组织缺损

摘　　要：目的探讨局部皮瓣在修复头面部皮肤软组织缺损的临床应用效果。方法选取应用局部皮瓣修复头面部病损切除术后皮肤软组织缺损的病人153例,根据病灶部位及大小选用合适的局部皮瓣进行Ⅰ期修复,术后观察瘢痕及有无畸形等情况,随访病人对修复的满意度。结果153例创面中,146例Ⅰ期愈合,7例创面出现并发症。术后随访6个月~1年,139例病人术区皮瓣颜色与周围皮肤无明显差异,切口线不明显,患处瘢痕无明显增生,眉毛、眼睑、鼻部及口角等部位无牵拉畸形等情况,病人对修复效果较为满意;有7例病人因手术切口线较长,局部瘢痕略明显而表示不满意。结论应用局部皮瓣进行头面部皮肤病损切除后皮肤软组织缺损的美容修复可获得良好的治疗效果,病人满意度高。Objective To investigate the clinical effect of local skin flaps in repairing skin and soft tissue defects of the head and face.Methods A total of 153 patients who underwent local flap repair of skin and soft tissue defects after resection of head and face lesions were enrolled.Local skin flaps were selected based on the location and size of the lesion and were used for stageⅠrepair,and scar and presence or absence of deformity were observed after surgery.The patients were followed up to observe the degree of satisfaction with repair.Results Of all 153 patients,146 achieved wound healing by first intention and 7 expe-rienced wound complications.The patients were followed up for 6 months to 1 year after surgery;139 patients had no significant difference in color between the flaps and the surrounding skin and the incision line was not obvious,with no marked proliferation of scar and no traction deformity in eyebrows,eyelids,nose,and corner of the mouth,and they were satisfied with the repair;7 patients were unsatisfied due to long surgical incision and obvious scars.Conclusion The application of local flaps can achieve a good therapeutic effect and a high degree of satisfaction in the cosmetic repair of skin and soft tissue defects after resection of skin lesions in the head and face.
