摘 要:目的探讨尿液中RNA氧化产物8-氧化鸟苷(8-oxo-Gsn)与丙型肝炎病毒(HCV)感染引起肝损害的关系及临床意义。方法选取45例慢性丙型肝炎(CHC)患者肝穿刺活检前的空腹中段晨尿,HCV携带者50例及50例体检健康者(对照组)的空腹中段晨尿,采用同位素稀释高效液相-串联质谱(ID-LC-MS/MS)检测尿RNA氧化产物8-oxo-Gsn,并用肌酐值(creatinine)校正。分析尿8-oxo-Gsn与CHC炎症程度和纤维化程度之间的相关性。结果CHC组尿8-oxo-Gsn水平显著高于HCV携带组(P<0.01);HCV携带组患者尿8-oxo-Gsn水平显著高于对照组(P<0.01);肝脏炎症程度高的患者(A2~A3)尿8-oxo-Gsn水平显著高于肝脏炎症程度低的患者(A0~A1),差异有统计学意义(P<0.01);纤维化程度高的患者(F3~F4)尿8-oxo-Gsn水平与纤维化程度低(F0~F2)的比较,差异无统计学意义(P>0.05)。结论尿中RNA氧化损伤标志物8-oxo-Gsn对评估丙型肝炎肝脏损害程度有较高的参考价值。Objective To investigate the relationship between urinary RNA oxidation product 8-oxo-Gsn and hepatic damage caused by hepatitis C virus(HCV)infection and its clinical significance.Methods The urine samples of 45 patients with chronic hepatitis C(CHC),50 hepatitis C carriers and 50 healthy controls were collected before liver biopsy.The urine RNA oxidized product 8-oxo-Gsn was detected by isotope dilution high performance liquid chromatography-tandem mass spectrometry(ID-LC-MS/MS)and corrected by creatinine value.To analyze the correlation between urinary 8-oxo-Gsn and the degree of inflammation and fibrosis of CHC.Results Urine 8-oxo-Gsn in CHC group was higher than that in hepatitis C carrier group(P<0.01);urine 8-oxo-Gsn in hepatitis C carrier group was significantly higher than that in healthy control group(P<0.01);urine 8-oxo-Gsn in patients with high degree of hepatitis(A2-A3)was significantly higher than that in patients with low degree of hepatitis(A0-A1),with significant difference(P<0.01);meanwhile,compared between patients with high degree of fibrosis(F3-F4)and patients with low fibrosis(F0-F2),there was no significant difference in urine 8-oxo-Gsn concentration(P>0.05).Conclusion Urinary RNA oxidative damage marker 8-oxo-Gsn has a good significance in evaluating the degree of liver damage in hepatitis C.
