间断水平褥式外翻缝合法在内科胸腔镜术中的应用

摘　　要：目的了解内科胸腔镜术中局部外切口行间断水平褥式外翻缝合法的临床应用价值。方法240例无禁忌证的行内科胸腔镜手术患者分为术中固定胸管后局部外切口行间断水平褥式外翻缝合法组(实验组,n=160)和不使用该方法缝合组(对照组,n=80)治疗,比较两组的术后住院时间、住院费用、并发症、随访3个月后伤口愈合瘢痕宽度。结果与对照组比较,实验组的术后住院时间短、住院费用少(P<0.05或0.01),术后并发气胸发生率低(P<0.01),术后随访3个月的伤口瘢痕宽度更小(P<0.01)。结论内科胸腔镜术中外切口应用间断水平褥式外翻缝合法,术后住院时间短,住院费用及术后并发症发生率更低,伤口愈合瘢痕宽度更小。Objective To explore the clinical application value of interrupted horizontal mattress suture for external incision in medical thoracoscopy.Methods A total of 240 patients with no contraindications who underwent medical thoracoscopy surgery were randomly divided into the Experimental Group(n=160)who received the interrupted horizontal mattress suture for external incision after intraoperative fixation of thoracic duct and Control Group(n=80)who received no interrupted horizontal mattress suture.The two groups were compared in terms of postoperative hospital stay,hospitalization expenses,complications and the width of scar after wound healing in 3-month follow-up.Results Compared with the Control Group,the Experimental Group had a shorter postoperative hospital stay and less hospitalization expenses(P<0.05 or 0.01),had a lower incidence of postoperative pneumothorax(P<0.01)and smaller scar of wound after 3 months of follow-up(P<0.01).Conclusion The application of interrupted horizontal mattress suture for external incision in thoracoscopic surgery is an effective method with less postoperative hospital stay,lower hospitalization expenses,lower incidence of postoperative complications,and smaller scar after wound healing.
