摘　　要：目的回顾性分析肱二头肌桡骨肌腱滑囊炎(BRB)的MRI表现,探讨MRI对BRB的诊断价值。方法对8例经手术病理证实的BRB患者行MRI检查,观察滑囊炎的大小、形态和信号等特征,并分析其征象。结果8例患者发现肘部肿物,患处有不同程度的疼痛及压痛,其中4例患者手部及前臂麻木不适感伴运动受限,经MRI及手术证实,2例患者正中神经受压,2例患者桡神经受压,受压神经增粗。6例患者肱二头肌桡骨肌腱大部被囊肿包绕,囊肿主体位于肌腱深部;2例患者肌腱局部被囊肿包绕,囊肿主要位于旋后肌间隙内,呈多房囊性改变。BRB囊壁厚度和边界清晰与否,与囊肿壁和周边炎症情况相关,滑囊均不与肘关节腔相通。结论在MRI图像上,BRB有显著的解剖学特征和影像学特点,结合临床表现,MR检查可对其可做出正确诊断。Objective To review imaging features of bicipitoradial?bursitis,and to explore the diagnostic value of MRI for this lesion.Methods 8 patients withthe bicipitoradial?bursitis were analyzed retrospectively.All cases were proved by surgical operation and pathological examination.MR imaging was performed in all patients,including 5 nonen-hanced scans and 3 contrast-enhanced examinations.Thesize,morphology and the signal feature were observed meticulous-ly.Results 8 patients had a palpable mass in the elbow,and had different degrees of pain and tenderness.4 of these pa-tients had limited movement of the hand and forearm,which correlated with nerve compression which was revealed at MR imaging and surgery,2 of the patients had radial nerve compression and 2 patients had median nerve compression,the nerve fibers were thickened.Of the 6 cases the biceps tendon was enveloped completely by bicipitoradial bursa,with differ-ent thickness of the cystic wall.Some different degreesinflammatoryprocess were revealed in cystic walls and the adjacent zone.Of 2 cases,the tendon was partially entraped by the bursa and the multilocular cyst invaded into the supinator.The cystic wall and septum were thin.All bursae wereisolated to the elbow joint cavity.Conclusion Bicipitoradial?bursit-ishad special clinical manifestations,significant anatomical features and imaging findings.MR imaging has an important role for evaluationof the bicipitoradial bursitis.
